摘要
目的探讨AGT、AGTR1、ATP2B1基因多态性及基因-环境交互作用与萧山区原发性高血压的关联性.方法选取原发性高血压患者100例(HTN组),正常血压对照组70例(对照组),使用美国Axygen公司试剂盒提取人血全基因组DNA,应用直接测序法检测位点rs699、rs5051、rs5186及rs12817819单核苷酸多态性,利用SPSS 19.0统计软件进行数据分析.结果HTN组腰围、体质量指数(BMI)、空腹血糖(FBG)、甘油三酯、尿酸、血肌酐水平与对照组差异存在统计学意义(P<0.05).经Logistic回归校正性别、年龄、BMI、血肌酐、同型半胱氨酸(Hcy)、尿酸等因素后,FBG(OR=6.012,95%CI:1.430~25.279,P<0.01)、总胆固醇(TC)(OR=16.078,95%CI:2.756~93.802,P<0.01)、腰围(OR=1.095,95%CI:1.006~1.191,P<0.05)为高血压独立危险因素,未发现基因多态性与高血压相关联(P>0.05).Logistic回归分析显示,FBG、TC、腰围交互作用与原发性高血压显著相关(OR=1.005,95%CI:1.002~1.008,P<0.01),校正性别、年龄、BMI、血肌酐、血Hcy、尿酸、吸烟等因素后,上述相关性仍有统计学意义(P<0.01).结论FBG、TC、腰围与萧山区人群原发性高血压患病显著相关,未发现目标基因位点多态性与本地区原发性高血压发病相关联.
Objective To investigate the association between polymorphism of AGT,AGTR1,ATP2B1 gene,their interaction with environment and susceptibility of primary hypertension in population of Xiaoshan area.Method 100 patients of primary hypertension and 70 ones of control group were enrolled.Genomic DNA was extracted from peripheral blood leukocytes,using the QIAamp DNA Mini Kit(Axygen,America).Four SNPs,rs699,rs5051,rs5186 and rs 12817819 were examined,using a direct sequencing method.Statistical analyses were performed with SPSS Windows software(version 19.0).Results A significant difierence was found in waistline,BM I,fasting glucose,blood triglyceride,serum creatinine and uric acid between cases and controls(P<0.05).After adjustment for age,sex,BM I,serum creatinine,uric acid and homocysteine,fasting glucose(OR=6.012,95%CI:1.430-25.279,P<0.01),cholesterol(OR=16.078,95%C I:2.756-93.802,P<0.01)and waistline(OR=l.095,95%CI:1.006〜1.191,P<0.05)were significantly associated with the incidence of primary hypertension.However,the association between polymorphism of AGT,AG TR1,ATP2B1 gene and the occurrence of primary hypertension was not significant(P>0.05).Logistic regression analysis showed that fasting glucose,cholesterol and waistline interaction was associated with the incidence of primary hypertension(OR=1.005,95%CI:1.002〜1.008,P<0.01),and the association was still existed after the adjustment for age,sex,B M I,homocysteine,serum creatinine,uric acid and smoking.Conclusion Fasting glucose,cholesterol and waistline are significantly associated with the incidence of primary hypertension in population of Xiaoshan area.The association between polymorphism of AGT,A G TR1,ATP2B1 gene and the incidence of primary hypertension is not found in this study.
