鼻咽癌中药超声雾化鼻吸入联合鼻腔冲洗预防放射性鼻窦炎的疗效观察

Observation on effect of TCM ultrasonic atomizing nasal inhalation combined with nasal irrigation on sinusitis after nasopharyngeal carcinoma radiotherapy

目的观察中药超声雾化鼻吸入联合鼻腔冲洗预防鼻咽癌放射治疗后鼻窦炎的临床疗效。方法选取符合纳入标准的鼻咽癌放射治疗患者116例,按随机数字表分为对照组58例和观察组58例。对照组从放射治疗开始至结束,每日予0.9%氯化钠注射液100 ml鼻腔冲洗;观察组在对照组的基础上加用中药超声雾化鼻吸入治疗。比较两组患者干预后第4周和第7周的临床症状积分,第7周的临床疗效及放射治疗3个月后的放射性鼻窦炎发生率。结果干预后第4周和第7周观察组患者头痛、鼻塞、流涕和嗅觉功能减退评分均显著优于对照组;第7周观察组临床疗效总有效率为96.6%,对照组为84.5%;放射治疗3个月后观察组放射性鼻窦炎发生2例(3.4%),对照组9例(15.5%);经比较差异均有统计学意义(P均<0.05)。结论中药超声雾化鼻吸入联合鼻腔冲洗,可改善鼻咽癌放射治疗患者的鼻窦炎临床症状,有效降低放射性鼻窦炎的发生率。

Objective To observe the clinical effect of TCM ultrasonic atomizing nasal inhalation combined with nasal irrigation on sinusitis after nasopharyngeal carcinoma radiotherapy.Method Choose 116 patients in accordance with the inclusion criteria after nasopharyngeal carcinoma radiotherapy and divide them into observation group(n=58)and control group(n=58)by random number table.Patients in control group receive nasal irrigation of 100ml normal saline.Patients in observation group receive TCM ultrasonic atomizing nasal inhalation based on nasal irrigation of normal saline.Compare the clinical symptom integrals after 4 and 7 weeks of intervention,clinical effect of the seventh week and incidence of sinusitis after 3 months of radiotherapy between two groups.Result Scores on headache,nasal obstruction,nasal discharge and olfactory function decrease of patients after 4 and 7 weeks of intervention are significantly better in observation group than in control group.The total effective rate of clinical effect in the seventh week is 96.6%in observation group and 84.5%in control group.There are 2 cases(3.4%)of sinusitis in observation group and 9 cases(15.5%)in control group after 3 months of radiotherapy.The difference is significant between two groups(all P<0.05).Conclusion TCM ultrasonic atomizing nasal inhalation combined with nasal irrigation can relieve clinical symptoms of sinusitis after nasopharyngeal carcinoma radiotherapy and reduce incidence of sinusitis.
