摘要
目的对新冠肺炎疫情下发热门诊医务人员心理状态调查,并分析总结心理特征,提出应对策略。方法纳入新冠肺炎疫情下参与发热门诊工作的118名医务人员和参与普通门诊工作的110名医务人员,分别设为发热门诊组和普通岗位组。采用自行设计的一般情况表采集基本信息,使用简易心理弹性量表(CD-RISC)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评价2组的心理状态,采用匹兹堡睡眠质量指数量表(PSQI)评价2组的睡眠质量。使用SPSS 20.0软件对数据进行统计分析,计量资料采用t检验,计数资料采用χ2检验。结果2组CD-RISC总评分比较差异有统计学意义(P<0.05);发热门诊组较普通门诊组评分更高,尤其是“坚韧性”评分,2组比较差异有统计学意义(P<0.05)。发热门诊组较普通门诊组在焦虑、抑郁情绪方面比较差异有统计学意义(P<0.05);发热门诊组医务人员焦虑、抑郁情绪较普通门诊组更轻。在PSQI评价中,2组总评分比较差异有统计学意义(P<0.05),其中“睡眠质量”“睡眠紊乱”“日间功能紊乱”3个因子方面比较差异均有统计学意义(P值均小于0.05)。结论发热门诊医务人员在心理应激下表现出睡眠质量差、睡眠节律紊乱、日间功能差等问题。为应对这些问题,医院管理部门应尽力提供给发热门诊医务人员舒适睡眠环境及睡眠指导。
Objective Investigated the psychological status of medical staff in hot outbreaks under the epidemic of COVID-19,analyzed and summarized the psychological characteristics,and propose response strategy.Methods The 118 medical staff who participated in the fever clinic work and the 110 medical staff who participated in the general outpatient work under the COVID-19 epidemic were set as the fever clinic group and the general post group,respectively.The self-designed general condition table was used to collect basic information.The simple mental resilience scale(CD-RISC),self-evaluation of anxiety scale(SAS),depression self rating scale(SDS)were used to evaluate the psychological status of the two groups,and the Pittsburgh sleep quality index scale(PSQI)was used to evaluate the sleep quality of the two groups.SPSS 20.0 software was used for statistical analysis of the data.The measurement data were tested by t test and the count data+were tested by chi-square test.Results There was a statistically significant difference in the total score of the CD-RISC between the two groups(P<0.05);the fever clinic group had a higher score than the general outpatient group,especially the“toughness”score,and the difference between the two groups was statistically significant(P<0.05).There was a statistically significant difference in anxiety and depression between the fever outpatient group and the general outpatient group(P<0.05);the anxiety and depression in the medical staff in the fever outpatient group were lighter than those in the general outpatient group.In the PSQI evaluation,there was a statistically significant difference in the total scores of the two groups(P<0.05).There were statistically significant differences in the three factors of“sleep quality”,“sleep disturbance”and“day dysfunction”(with P values below 0.05).Conclusion Fever medical staff under psychological stress showed problems such as poor sleep quality,sleep rhythm disorders,and poor daytime function.In order to cope with these psychological problems,the hospital management should try its best to provide comfortable sleeping environment and sleep guidance for the medical staff of the hot clinic.
