综合脑保护方案对老年急性脑梗死患者脑氧代谢及血管内皮功能的影响

Effects of comprehensive cerebral protection regimen on cerebral oxygen etabolism and vascular endothelial function in elderly patients with acute cerebral infarction

目的分析综合脑保护方案对老年急性脑梗死患者脑氧代谢及血管内皮功能的影响。方法选取2015年1月至2018年12月我院收治的336例急性脑梗死患者作为研究对象,按随机数字法将其分为观察组(168例,综合脑保护治疗)和对照组(168例,常规治疗)。比较两组治疗前、后的氧代谢指标、血液流变学指标及血管内皮功能指标。结果治疗后,两组的Da-jvO2、ERO2均下降,SjvO2、SaO2均升高,且观察组均优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的全血黏度、血浆黏度、全血还原黏度均降低,且观察组均低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的ET和ICP均降低,NO均升高,且观察组优于对照组,差异具有统计学意义(P<0.05)。结论综合脑保护方案可有效改善老年急性脑梗死患者的脑氧代谢、血管内皮功能及血液流变学,值得临床推广应用。

Objective To analyze the effects of comprehensive cerebral protection regimen on cerebral oxygen metabolism and vascular endothelial function in elderly patients with acute cerebral infarction.Methods A total of 336 patients with acute cerebral infarction enrolled in our hospital from January 2015 to December 2018 were selected as the study object and divided into observation group(168 cases,comprehensive cerebral protection treatment)and control group(168 cases,regular treatment)according to the random number method.The indexes of oxygen metabolism,hemorheology and vascular endothelial function of the two groups before and after treatment were compared.Results After treatment,the Da-jvO2 and ERO2 in the two groups decreased,SjvO2 and SaO2 increased,and those in the observation group were better than the control group(P<0.05).After treatment,the whole blood viscosity,plasma viscosity and whole blood reducing viscosity in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,ET and ICP in the two groups decreased,NO increased,and those in the observation group were better than the control group(P<0.05).Conclusion Comprehensive cerebral protection regimen can effectively improve cerebral oxygen metabolism,vascular endothelial function and hemorheology in elderly patients with acute cerebral infarction,which is worthy of clinical promotion and application.
