玻璃体腔注射康柏西普联合激光光凝治疗视网膜静脉阻塞继发黄斑水肿的效果

Effect of intravitreal injection of conbercept combined with laser photocoagulation on macular edema secondary to retinal vein occlusion

目的评价玻璃体腔注射康柏西普联合激光光凝治疗视网膜静脉阻塞(RVO)继发黄斑水肿(ME)的效果。方法回顾性分析2018年6月至2019年6月我院收治的196例(196眼)RVO继发ME患者资料,按照治疗方式的不同将其分为对照组与观察组,每组98例(98眼)。对照组进行激光光凝治疗,观察组在对照组基础上联合玻璃体腔注射康柏西普治疗。比较两组治疗效果。结果治疗后,两组BCVA、CMT、IOP均改善,且观察组优于对照组(P<0.05)。治疗后,两组VEGF、SDF-1及PDGF水平均降低,且观察组低于对照组(P<0.05)。观察组治疗总有效率为93.88%,高于对照组的82.65%(P<0.05)。两组随访期间均未发生视网膜脱落、眼内炎等严重并发症。治疗后,两组CLVQOL评分均升高,且观察组高于对照组(P<0.05)。结论玻璃体腔注射康柏西普联合激光光凝治疗RVO继发ME效果显著,可有效提高患者视力,稳定眼内压,降低VEGF、PDGF及SDF-1表达,提高患者生活质量。

Objective To evaluate the effect of intravitreal injection of conbercept combined with laser photocoagulation in the treatment of macular edema(ME)secondary to retinal vein occlusion(RVO).Methods The data of 196 cases(196 eyes)of ME secondary to RVO in our hospital from June 2018 to June 2019 were analyzed retrospectively.According to the different treatment methods,the patients were divided into control group and observation group,with 98 cases(98 eyes)in each group.The control group was treated with laser photocoagulation,the observation group was treated with intravitreal injection of conbercept on the basis of the control group.The treatment effects of the two groups were compared.Results After treatment,BCVA,CMT and IOP of the two groups improved,and those of the observation group were better than the control group(P<0.05).After treatment,the levels of VEGF,SDF-1 and PDGF in the two groups decreased,and those of the observation group were lower than the control group(P<0.05).The total effective rate of treatment in the observation group was 93.88%,which was higher than 82.65%of the control group(P<0.05).There were no serious complications such as retinal detachment and endophthalmitis occurred in the two groups during the follow-up period.After treatment,the CLVQOL score of the two groups increased,and that of the observation group was higher than the control group(P<0.05).Conclusion The intravitreal injection of conbercept combined with laser photocoagulation on ME secondary to RVO is significant.It can effectively improve vision,stabilize intraocular pressure,reduce the expression of VEGF,PDGF and SDF-1,and improve the quality of life of patients.
