血清乳酸与脑钠肽联合检测对小儿重症手足口病的诊断价值 被引量:4

摘要 目的:探讨血清乳酸(LAC)与脑钠肽(BNP)检测在重症手足口病患儿中的诊断价值。方法:回顾性分析115例手足口病患儿的临床资料,作为研究组;选取同期到本院体检的50例健康儿童,作为对照组。研究组根据临床表现分为轻症、重症重型、重症危重型。两组均检测血清LAC、BNP,比较组间差异。结果:研究组血清LAC、BNP高于对照组,且研究组重症重型、重症危重型高于轻症,重症危重型高于重症重型(P<0.05);Pearson相关分析显示,重症手足口病患儿血清LAC、BNP水平呈正相关(P<0.05);血清LAC、BNP联合诊断重症手足口病时,ROC曲线下面积0.912,敏感度81.74%,特异度97.39%,准确度87.83%。结论:重症手足口病患儿诊断中血清LAC、BNP联合检测价值较高。 Objective:To investigate the diagnostic value of serum lactic acid(LAC)and brain natriuretic peptide(BNP)in children with severe hand-foot-mouth disease(HFMD).Methods:115 children with hand-foot-mouth disease were selected as study group,and 50 healthy children during the same period were selected as control group.According to the clinical manifestations,the study group was divided into mild type,severe type and critical type.Serum LAC and BNP were detected in both groups and compared between the two groups.Results:The levels of serum LAC and BNP in the study group were higher than those in the control group,and which of the severe and critical types in the study group were higher than those in the mild type,and which of the critical type was higher than that in the severe type(P<0.05).Pearson correlation analysis showed that serum LAC and BNP levels in children with severe HFMD were positively correlated(P<0.05).When serum LAC and BNP were used to diagnose severe HFMD,the area under ROC curve was 0.912,the sensitivity was 81.74%,the specificity was 97.39%,and the accuracy was 87.83%.Conclusion:The combined detection of serum LAC and BNP is of high value in the diagnosis of severe HFMD.
