摘要
目的:探讨恶性淋巴瘤(ML)患者外周血β2微球蛋白(β2-MG)、乳酸脱氢酶(LDH)、神经特异性烯醇化酶(NSE)的表达水平及临床意义。方法:回顾性分析2017-05-2019-05收治的ML患者78例作为观察组,以同期进行健康体检且一般资料与观察组均衡可比的健康者20例作为对照组。比较观察组和对照组血清β2-MG、LDH、NSE含量差异。根据随访情况,将78例ML患者分为死亡组和存活组,比较死亡组和存活组的β2-MG、LDH、NSE表达水平差异。通过ROC曲线下的AUC面积评价血清β2-MG、LDH、NSE指标在预测ML患者预后。结果:观察组的β2-MG、LDH、NSE表达水平均明显高于对照组,差异有统计学意义(P<0.05)。根据2年的随访情况,出现死亡23例(29.49%),存活55例(70.51%),死亡组的β2-MG、LDH、NSE表达水平明显高于存活组,差异有统计学意义(P<0.05)。ROC曲线分析发现,β2-MG最佳截断值为4.42 mg/L(灵敏度=81.70,特异性=92.20),LDH最佳截断值393.68 U/L(灵敏度=69.80,特异性=84.60),NSE最佳截断值24.73 ng/mL(灵敏度=77.50,特异性=88.10)。结论:ML患者外周血β2-MG、LDH及NSE水平均呈高表达,与ML患者病情的发生、发展及预后密切相关。在临床管理中,检测ML患者的外周血β2-MG、LDH、NSE指标,有助于ML的早期诊断及预后防治。
Objective:To investigate the expression ofβ2-MG,LDH and NSE in peripheral blood of patients with malignant lymphoma and its clinical significance.Method:A retrospective analysis was made.78 ML patients treated in our hospital from May 2017 to May 2019 were taken as observation group,and 20 healthy persons with the same period of physical examination and comparable general data with observation group were taken as control group.The serum levels ofβ2-MG,LDH and NSE were compared between the observation group and the control group.According to the follow-up,78 patients with ML were divided into death group and survival group.The expression levels ofβ2-MG,LDH and NSE in death group and survival group were compared.Serumβ2-MG,LDH and NSE were evaluated by AUC area under ROC curve to predict the prognosis of ML patients.Result:The expression levels ofβ2-MG,LDH and NSE in the observation group were significantly higher than those in the control group(P<0.05).According to one-year follow-up,23 cases died(29.49%)and 55 cases survived(70.51%).The expression levels ofβ2-MG,LDH and NSE in the death group were significantly higher than those in the survival group(P<0.05).ROC curve analysis showed that the best cut-off value ofβ2-MG was 4.42 mg/L(sensitivity=81.70,specificity=92.20),LDH 393.68 U/L(sensitivity=69.80,specificity=84.60),and NSE 24.73 ng/mL(sensitivity=77.50,specificity=88.10).Conclusion:The levels of beta 2-MG,LDH and NSE in peripheral blood of patients with ML were all highly expressed,which was closely related to the occurrence,development and prognosis of ML.In clinical management,the detection of peripheral blood beta 2-MG,LDH and NSE in patients with ML is helpful to the early diagnosis and prognosis of ML.
