中医骨伤护理对全髋关节置换术后疼痛及功能的影响

Therapeutic Effect of Traditional Chinese Medicine Bone Injury Nursing on Postoperative Pain and Function after Total Hip Arthroplasty

目的:探讨中医骨伤护理对全髋关节置换术后疼痛及功能的护理疗效。方法:将80例全髋关节置换术后患者按随机数字表法均分为治疗组和对照组各40例,对照组采用常规护理,治疗组在此基础上配合中医骨伤护理,包括中医情志护理和穴位按揉,比较两组患者对护理工作的满意度、VAS评分和髋关节功能评分,并将护理工作满意度分别与VAS评分和Harris评分进行相关性分析。结果:术后12h,两组VAS评分差异无统计学意义(P>0.05),术后24、48h时,治疗组VAS评分均显著低于对照组,差异有统计学意义(P<0.05);干预前,两组Harris评分差异无统计学意义(P>0.05),干预后2周治疗组Harris评分显著高于对照组,差异有统计学意义(P<0.05);治疗组护理工作满意度显著优于对照组(P<0.05);护理工作满意度与术后24h的VAS评分呈显著负相关(P<0.05),与干预后2周Harris评分呈显著正相关(P<0.05)。结论:对全髋关节置换术后疼痛患者进行中医骨伤护理取得了显著疗效,未来在临床上值得广泛推广,且护理工作满意度与疼痛评分和髋关节功能的改善密切相关。

Objective:To explore the nursing effect of TCM bone injury nursing on pain and function after total hip arthroplasty.Method:80 patients with total hip arthroplasty were divided into treatment group(n=40)and control group(n=40)according to the random number table.The control group received routine nursing,while treatment group was combined with traditional Chinese medicine bone injury nursing,including Chinese medicine,emotional care and acupuncture points.The satisfaction of nursing work,VAS score and hip function were statistically analyzed in two groups,and the correlation between nursing job satisfaction and VAS score and Harris score were analyzed.Result:There was no significant difference in VAS score between the two groups at 12 hours after operation(P>0.05).The VAS scores of the treatment group were significantly lower than those of the control group at 24 and 48 hours after operation(P<0.05).There was no significant difference in Harris score between the two groups(P>0.05).The Harris score of the treatment group was significantly higher than that of the control group at 2 weeks after intervention(P<0.05).The satisfaction of nursing work in the treatment group was excellent.In the control group(P<0.05),there was a significant negative correlation between nursing job satisfaction and VAS score at 24 h postoperatively(P<0.05),and a significant positive correlation with Harris score at 2 weeks after intervention(P<0.05).Conclusion:In the study of traditional Chinese medicine bone injury nursing for patients with pain after total hip arthroplasty,significant clinical efficacy has been achieved,which is worthy of widespread promotion in the future,and the satisfaction of nursing work and the improvement of pain score and hip function closely related.
