经脐单孔腹腔镜技术在妇科中的临床应用进展

Advances in the application of transumbilical single-site laparoscopy in gynecology

经脐单孔腹腔镜手术(transumbilical laparoendoscopic single-site surgery,TU-LSSS)是当下新兴的手术方式,近年来在妇科手术中被推广应用。TU-LSSS相较于传统腔镜手术具有手术切口美观、减轻术后疼痛、预防肿瘤种植等优势。TU-LSSS因其术中器械操作平面交叉、手术视野局限、三角关系缺失等技术难点限制其广泛开展,依赖手术操作器械创新,应用"筷子法"等手术操作技巧有助于克服手术中的困难。目前在妇科多用于良性肿瘤,在恶性肿瘤及盆底障碍性疾病中应用较少,并取得良好应用发展潜力,可能是未来妇科手术的重要发展方向。

The transumbilical laparoendoscopic single-site surgery(TU-LSSS)is an emerging way of operation,has been popularized in gynecologic surgery recently.Compared with traditional endoscopic surgery,TU-LSSS has the advantages of beautiful incision,alleviatepostoperative pain,and preventing tumor implantation.TU-LESS is limited in its extensive development due to technical difficulties such as intersections of operating surfaces,limited surgical field of vision,and inaccurate triangular relations.It relies on innovation of operating instruments,and the application of"chopstick method"and other operating techniques can help overcome the difficulties in operation.At present,it is mostly used in benign tumors,but less in malignant tumors and pelvic floor disorders,and has good application potential,which may be an important development direction of gynecological surgery in the future.
