游泳训练辅助治疗新生儿支气管肺发育不良的临床观察 被引量:2

摘要 目的研究并探讨游泳训练辅助治疗新生儿支气管肺发育不良(bronchopulmonary dysplasia,BPD)的临床效果,为更有效的治疗BPD提供参考依据。方法选择符合纳入标准的2012年1月1日至2018年12月31日十堰市人民医院新生儿科收治的BPD患儿70例,将其随机分为对照组和观察组,每组各35例,对照组采用常规治疗方案,观察组在常规治疗的基础上加用呼吸功能训练辅助治疗,比较两组患儿的临床症状改善情况、动脉血气分析、吸氧时间、住院时间及相关早产儿并发症发生率。结果经过2 w治疗后,观察组患儿总有效率达91.43%,明显高于对照组,两组差异具有统计学意义(P<0.05);两组患儿的PaO2、PaCO2均较治疗前显著改善,且观察组治疗2 w后的PaO2、PaCO2均优于对照组患儿,两组差异具有统计学意义(P<0.05)。观察组患儿的吸氧时间、住院时间、全肠内营养时间均短于对照组患儿,两组差异具有统计学意义(P<0.05)。观察组与对照组患儿出生时身长、体重比较差异无统计学意义(P>0.05),纠正胎龄40w时观察组身长、体重均高于对照组,且两组差异均具有统计学意义(P<0.05)。结论游泳训练辅助治疗有助于BPD患儿具有显著的临床疗效,可以改善患儿血气分析状况,缩短氧依赖时间,减少住院时间,促进生长发育,具有重要的临床应用价值。 Objective To study and explore the clinical effects of swimming training in the treatment of bronchopulmonary dysplasia(BPD),and provide a reference for more effective treatment of BPD.Methods A total of 70 children with BPD admitted to the department of neonatology,Shiyan City people s Hospital from January 1,2012 to December 31,2018 were selected.The patients were randomly divided into the control group and the observation group,with 35 cases in each group.The control group was treated with conventional treatment.The observation group was supplemented with respiratory function training on the basis of routine treatment.The clinical symptoms improvement,arterial blood gas analysis,oxygen inhalation time,hospitalization time and the incidence of related complications of preterm infants were compared between the two groups.Results After two weeks of treatment,the total effective rate of the observation group was 91.43%,which was significantly higher than that of the control group.The difference between the two groups was statistically significant(P<0.05).The PaO2 and PaCO2 of the two groups were significantly improved compared with those before treatment.PaO2 and PaCO2 in the observation group were better than those in the control group after two weeks of treatment.The difference between the two groups was statistically significant(P<0.05).The oxygen inhalation time,hospitalization time,and total enteral nutrition time of the observation group were shorter than those of the control group.The difference between the two groups was statistically significant(P<0.05).There was no significant difference in body length and body weight between the observation group and the control group(P>0.05).The length and weight of the observation group were greater than those of the control group at the time of corrected gestational age of 40 weeks,and the difference between the two groups was statistically significant(P<0.05).Conclusion Assisted swimming training has a significant clinical effect on children with BPD.It can improve blood gas analysis,shorten oxygen dependence time,reduce hospital stay,promote growth and development,and therefore it has important clinical application value.
