临床患者红系检测结果与昼夜节律相关性分析 被引量:1

摘要 目的评估昼夜变化对红细胞系测定值的影响并分析其原因,为贫血患者择时采血与择时输血提供理论依据。方法从我院住院且24 h内分别于2:00±2 h及14:00±2 h采血并进行血细胞分析的患者(无出血、输血及大量输液)中,随机抽取30例血红蛋白(hemoglobin,HGB)含量正常患者(男≥120 g/L,女≥110 g/L)、30例轻度贫血患者(HGB含量为90~110 g/L)、30例中重度贫血患者(HGB含量为50~90 g/L)的血细胞分析结果进行统计学分析。结果所有患者的红细胞(red blood cell,RBC)、HGB、红细胞比容(hematocrit,HCT)在14:00±2 h的测定结果均高于2:00±2 h的测定结果;与HCT的测定结果相比较,RBC及HGB的测定结果变化更为明显;3组患者中,以中重度贫血患者组红细胞系测定结果随昼夜变化最为显著,有明显的统计学意义(P<0.05)。结论人外周血红细胞系的测定结果受昼夜节律变化影响,以中重度贫血患者最为显著。 Objective To evaluate the effect of circadian variation on erythrocyte detection and to analyze its causes,provide a theoretical basis for the selection of blood sampling and timing of blood transfusion in patients with anemia.Methods Patients(no bleeding,blood transfusion and infusion)who adopted blood collection and blood cell analysis at 2:00±2 h and 14:00±2 h respectively in 24 hours in our hospital.The blood cell analysis results of 30 patients with normal hemoglobin(hemoglobin,HGB)content(male≥120 g/L,female≥110),30 patients with mild anemia(HGB content:90~110 g/L)and 30 patients with moderate and severe anemia(HGB content:50~90 g/L)were randomly selected for statistical analysis.Results All patients results of red blood cells(RBC),HGB,RBC hematocrit(HCT)detected at 14:00±2 h were higher than at 2:00±2 h,and compared with hematocrit(HCT)results,the change of test results of RBC and HGB was more obvious;in three groups of patients,effect of circadian variation on the erythrocyte detection results in patients with moderate and severe anemia was the most significant,which was statistically significant(P<0.05).Conclusion Human peripheral blood cells test results are affected by circadian rhythms,especially among those with moderate-severe anemia.
