血流向量成像技术评价房间隔缺损的左室流场变化特点及能量损耗变化规律

Evaluation of Changes in Left Ventricular Flow Field and Changes in Energy Loss of Atrial Septal Defect by Blood Flow Vector Imaging

摘要 目的应用血流向量成像技术评估房间隔缺损左室流场的变化特点及能量损耗的变化规律,为定量评估ASD左室结构和功能提供新的参考。方法选取ASD患者60例,分为合并肺动脉高压(PAH)组32例和无PAH组28例,健康志愿者31例为正常对照组,利用VFM技术测量左室涡流相关参数、EL值大小,分析总结相关演变规律。结果①左室流线成像均显示在舒张早期的快速充盈期(P1)、舒张中期(P2),二尖瓣口附近存在较多涡流。②合并PAH时各时相下的涡旋面积、循环强度、涡旋数量、EL值明显增大;EL峰值均出现在P1期(P<0.05)。结论VFM成像技术可显示ASD左室内血流的流场状态变化特点以及能量损耗的变化规律,为定量评估ASD左室流体力学相关变化及其心功能指数提供有力的参考。 Objective Evaluating the characteristics of left ventricular flow field and EL in ASD by using VFM provides a new reference for quantitative assessment of left ventricular structure and function in ASD.Methods Sixty patients with ASD were divided into two groups:32 patients with PAH group and 28 patients without PAH group.Thirty-one healthy volunteers were in normal control group.Left ventricular eddy current parameters and EL were measured by VFM,and the related evolution laws were analyzed and summarized.Results①Streamline imaging showed that there were more eddies near the mitral valve orifice in P1 and P2.②When PAH was merged,the area of vortices,cycle strength,number of vortices and EL increased significantly in each phase,and EL peak appeared in P1(P<0.05).Conclusion VFM imaging technology can display the characteristics of flow field and EL of left ventricular blood flow in ASD.It can provide a powerful reference for quantitative evaluation of left ventricular fluid dynamics and cardiac function index in ASD.
