胸部物理治疗对ICU昏迷患者肺部感染的临床观察

Effect of Chest Physiotherapy on Pulmonary Infection of Comatose Patients in ICU

目的:探讨胸部物理治疗对重症医学科(ICU)昏迷患者肺部感染的影响。方法:选择2018年1月-2019年6月湘雅医院重症医学科收治的80例昏迷合并肺部感染患者,按随机数字表法分为对照组与观察组,各40例,对照组采用常规治疗方案,观察组在常规治疗的基础上进行胸部物理治疗干预,对比两组治疗前后外周血白细胞(WBC)计数、C-反应蛋白(CRP)、氧合指数、临床肺部感染评分(clinical pulmonary infection score,CPIS)和治疗效果。结果:治疗后两组外周血WBC计数、CRP、CPIS均低于治疗前,氧合指数均高于治疗前,且观察组指标均优于对照组,差异均有统计学意义(P<0.05);观察组治疗总有效率为70.00%,高于对照组的42.50%,差异有统计学意义(P<0.05)。结论:胸部物理治疗可提高重症医学科昏迷患者肺部感染治疗效果,值得在临床上推广应用。

Objective:To explore the effect of chest physiotherapy on pulmonary infection of comatose patients in intensive care unit.Method:From January 2018 to June 2019,80 comatose patients with pulmonary infection admitted to the intensive care unit of Xiangya Hospital were selected,and were randomly divided into the control group and the observation group according to the random number table,with 40 cases in each group.The control group was treated with routine therapy,and the observation group was treated with chest physiotherapy on the basis of routine therapy.The white blood cell(WBC)count,C-reactive protein(CRP),oxygenation index and clinical pulmonary infection score(CPIS)of the two groups before and after treatment were compared.The effective rate of the two groups were compared after treatment.Result:After treatment,the WBC count of peripheral,bloodand CRP,CPIS of the two groups were lower than those before treatment,the oxygenation index was higher than that before treatment,and the indexes of the observation group were better than those of the control group,the differences were statistically significant(P<0.05).The effective rate of the observation group was 70.00%,higher than 42.50%of the control group,the difference was statistically significant(P<0.05).Conclusion:Chest physiotherapy can improve the total effective rate of the treatment of pulmonary infection in comatose patients in the intensive care unit,it is worthy to spread in clinic.
