PCT、CRP、IL-6、D-D在小儿脓毒症血流感染及非血流感染疾病中的诊断价值

Diagnostic Value of PCT,CRP,IL-6,D-D in Sepsis Children with Bloodstream Infection and Non Bloodstream Infection

目的:探究降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素-6(IL-6)及D-二聚体(D-D)在小儿脓毒症血流感染及非血流感染疾病中的诊断价值。方法:回顾性选取2016年1月-2019年1月闽东医院收治的脓毒症患儿95例,其中血流感染51例,非血流感染44例。收集患儿血清PCT、CRP、IL-6及D-D水平的相关资料并进行比较,采用受试者工作特征曲线(ROC)分析上述指标的诊断效能。结果:血流感染组血清PCT、CRP、IL-6、D-D水平较非血流感染组明显高,差异均有统计学意义(P<0.05);PCT、CRP、IL-6、D-D及联合检测的AUC分别为0.904、0.750、0.716、0.799及0.924,联合检测诊断效能高于单项检测。结论:PCT、CRP、IL-6及D-D在小儿脓毒症血流感染及非血流感染疾病中有一定的诊断价值,采用四项联合检测可提高血流感染的诊断效能。

Objective:To investigate the diagnostic value of procalcitonin (PCT),C-reactive protein (CRP),interleukin-6 (IL-6) and D-Dimer (D-D) in sepsis children with bloodstream infection and non bloodstream infection.Method:A total of 95 children with sepsis admitted to Mindong Hospital from January 2016 to January 2019 were retrospectively selected,including 51 cases with bloodstream infection and 44 cases with non bloodstream infection.The relevant data on serum PCT,CRP,IL-6 and D-D levels of children were collected and compared,and the diagnostic efficacy of the above indexes were analyzed by receiver operating characteristic curve (ROC).Result:The levels of PCT,CRP,IL-6 and D-D in the bloodstream infection group were significantly higher than those of the non bloodstream infection group,and the differences were statistically significant (P<0.05).The AUC of PCT,CRP,IL-6,D-D and combined detection were 0.904,0.750,0.716,0.799 and 0.924 respectively,and the diagnostic efficacy of the combined detection was higher than that of the single detection.Conclusion:PCT,CRP,IL-6 and D-D have a certain diagnostic value in sepsis children with bloodstream infection and non bloodstream infection.The combined detection of four items can improve the diagnostic efficacy of bloodstream infection.
