25R胰岛素增敏剂对磺脲类药物继发失效T2DM患者的影响

Effect of Youmile 25R combined with insulin sensitizer on type 2 diabetes mellitus patients with secondary sulfonylureas failure

目的分析精蛋白锌重组赖脯胰岛素混合注射液(25R)胰岛素增敏剂对磺脲类药物继发失效2型糖尿病(T2DM)患者血糖控制及SF-36评分的影响。方法选取我院收治的对磺脲类药物继发失效T2DM患者74例,随机分为观察组和对照组,每组37例。对照组予以吡格列酮片、甘精胰岛素,观察组予以吡格列酮片+25R,比较两组治疗前后空腹血糖(FBG)、早中晚餐后2 h血糖(2h PG)、糖化血红蛋白(HbA1c)水平、SF-36评分、C反应蛋白(CRP)、低血糖发生率。结果观察组治疗后早中晚2 h PG、HbA1c低于对照组(P<0.05);两组治疗后SF-36评分均高于治疗前,CRP均低于治疗前,且观察组改善幅度大于对照组(P<0.05);观察组低血糖发生率与对照组相比,P>0.05。结论 25R+胰岛素增敏剂应用于对磺脲类药物继发失效2型糖尿病,可有效控制患者血糖,改善患者机体炎症反应,提高患者生活质量。

Objective To analyze the effect of protamine zinc recombinant lysine insulin mixed injection(Youmile 25) cimbined with insulin sensitizer on the blood glucose control and SF-36 of type 2 diabetes mellitus(T2 DM) patients with secondary sulfonylureas failure. Methods Seventy-four T2 DM patients with secondary sulphonylurea failure were admitted to our hospital and were randomly divided into the observation group and the control group, 37 cases in each group. The control group received pioglitazone tablets combined with insulin glargine and the observation group received pioglitazone tablets combined with Youmile 25. The fasting blood glucose(FBG), blood glucose 2 h after meals(2 h PG), glycosylated hemoglobin(HbA1 c) level, SF-36 score, C-reactive protein(CRP) and incidence of hypoglycemia were compared between the two groups before and after treatment. Results After treatment, the 2 h PG and HbA1 c in the observation group were lower than those in the control group(P<0.05). The SF-36 scores in the two groups were higher than those before treatment, the CRP was lower than that before treatment, and the improvement of the observation group was greater than that of the control group(P<0.05);the incidence of hypoglycemia in the observation group was not significantly different from that in the control group(P>0.05). Conclusion Youmile 25 combined with insulin sensitizer can effectively control the blood glucose, improve the body’s inflammatory response and improve the quality of life of T2 DM patients with secondary sulphonylurea failure.
