小儿推拿辅助治疗儿童急性鼻-鼻窦炎26例临床观察

Clinical observation on 26 cases of children with acute rhinitis-nasosinusitis treated by pediatric tuina as adjunctive therapy

目的观察小儿推拿辅助治疗儿童急性鼻-鼻窦炎的临床疗效。方法选取2018年10月至2019年4月在孝感市妇幼保健院中医儿科门诊就诊的急性鼻-鼻窦炎患儿52例,按照随机数字表法分为对照组和治疗组,各26例。对照组予头孢呋辛酯颗粒口服、生理盐水鼻腔冲洗,治疗组在对照组治疗方法的基础上予小儿推拿祛风清热、化浊通窍,2组均治疗2周后统计疗效。结果治疗前2组单项症状积分和总积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组上述积分均显著下降,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组下降更显著(P<0.05)。对照组总有效率为53.8%(14/26),治疗组为76.9%(20/26),2组比较,差异有统计学意义(P<0.05)。治疗后3个月,对照组复发率为50.0%(7/14),治疗组为10.0%(2/20),2组比较,差异有统计学意义(P<0.05)。结论小儿推拿辅助治疗儿童急性鼻-鼻窦炎,不仅疗效显著,复发率低,而且无痛苦,患儿易接受,值得临床推广应用。

Objective To observe the clinical efficacy of pediatric tuina in adjunctive treatment of children with acute rhinitis-nasosinusitis.Methods Fifty-two cases of acute rhinitis-nasosinusitis consulted in pediatric department of TCM of Xiaogan Maternal and Child Health Care Hospital from October 2018 to April 2019 were divided into control group and treatment group according to random number table method, with 26 cases in each.The control group was given oral administration of cefuroxime axetil granules and irrigation of nasal cavity with normal saline,on this basis,the treatment group was given pediatric tuina of expelling wind and clearing away heat as well as resolving turbidity and dredging orifices.We kept statistics about the curative effects after the 2 groups were both treated for 2 weeks.Results Before treatment,the difference was not statistically significant in single symptom score and total score between the 2 groups(P>0.05),and they were comparable.After treatment,the above-mentioned score of the 2 groups all significantly decreased,and the difference was statistically significant compared with the same group before treatment(P<0.05),moreover,the treatment group decreased more significantly(P<0.05).The total effective rate of the control group was 53.8%(14/26),that of the treatment group was 76.9%(20/26),and the difference was statistically significant between the 2 groups(P<0.05).After treatment for 3 months, the recurrence rate of the control group was 50.0%(7/14),that of the treatment group was 10.0%(2/20),and the difference was statistically significant between the 2 groups(P<0.05).Conclusion It has marked curative effects,low recurrence rate,no pain and easy acceptance for children patients to use pediatric tuina in adjunctive treatment of children with acute rhinitis-nasosinusitis,which deserves clinical promotion and application.
