药学对口支援促进县级医院抗菌药物合理使用的效果分析

Effect Analysis of Pharmacy Partner Assistance on the Rational Use of Antibiotics in a County-level Hospital

目的探讨药师对口支援促进受援医院抗菌药物合理使用的工作方法,评价支援前后受援医院抗菌药物临床应用各项考核指标的变化。方法针对受援医院抗菌药物管理使用存在问题,支援药师带领医务人员完成建立健全抗菌药物管理制度,加强培训,制定临床科室考核指标,开展抗菌药物处方、病历点评等工作。结果住院患者抗菌药物使用率,门、急诊患者抗菌药物处方比例,住院患者抗菌药物使用强度等与药学支援之前比有所下降,其中住院患者抗菌药物使用强度从60.63 DDDs/100人×天下降至42.63 DDDs/100人×天,下降明显。结论药学专业人才缺乏,医务人员抗菌药物知识匮乏是造成医院抗菌药物考核指标不达标重要因素,药学对口支援工作能够促进县级医院抗菌药物合理使用,提高医院药学服务水平。

Objective To explore the working methods of pharmacists’ partner assistance to promote the rational use of antibiotics in hospitals receiving assistance.Method The supporting pharmacist leaded medical staff to complete the establishment and improvement of antibiotics management system,strengthen training,develop clinical department assessment index,carry out antibiotic prescriptions and medical record reviews,etc.Results The utilization rate of antibiotics in inpatients,the proportion of antimicrobial agents prescriptions in outpatients and emergency department patients,and the antibacterial use density in inpatients decreased compared with that before pharmacists’ partner assistance.Among them,the antibacterial use density in inpatients decreased from 60.63 DDDs/100 patients×days to 42.63 DDDs/100 patients×days,with a significant decrease.Conclusion The shortage of pharmaceutical professionals and the lack of knowledge about antibiotics among medical staff are the important factors that lead to the substandard evaluation indexes of antibiotics in hospitals.Pharmacists’ partner assistance can promote the rational use of antibiotics in county-level hospitals and improve the level of pharmaceutical care in hospitals.
