ALA-PDT联合冷冻治疗老年头面部非黑色素瘤性皮肤癌及癌前病变的临床分析

Clinical analysis of ALA-PDT combined with cryotherapy for non melanoma skin cancer and precancerouslesions on the head and face of the elderly patients

目的分析5-氨基酮戊酸光动力疗法(ALA-PDT)联合冷冻治疗老年头面部非黑色素瘤性皮肤癌及癌前病变的近期疗效及满意度,为治疗该病症提供临床参考。方法选择2015年1月-2018年12月遂宁市中心医院收治的老年头面部非黑色素瘤性皮肤癌及癌前病变患者100例,依照数字表法随机分为观察组(n=50)与对照组(n=50)。对照组仅给予ALA-PDT治疗,观察组则行ALA-PDT配合冷冻治疗。治疗3个月后,观察并比较2组近期疗效、患者的治疗满意度、治疗不良反应和生活质量。结果治疗3个月,观察组的近期总有效率明显高于对照组(88.00%vs 74.00%,P<0.05);观察组的患者治疗满意度明显高于对照组(96.00%vs 80.00%,P<0.05);观察组的不良反应发生率明显低于对照组(12.00%vs 28.00%,P<0.05);观察组SF-36评分明显高于对照组[(70.52±10.13)vs(59.42±8.17),P<0.05]。结论 ALA-PDT联合冷冻治疗老年头面部非黑色素瘤性皮肤癌及癌前病变,近期疗效显著,患者治疗满意度高,不良反应少,生活质量改善更明显,有一定的临床应用价值。

Objective To analyze the short-term efficacy and satisfaction of 5-aminolevulinic acid photodynamic therapy(ALA-PDT)combined with cryotherapy in the treatment of non melanoma skin cancer and precancerous lesions on the head and face of the elderly patients,so as to provide clinical reference for the treatment of the disease.Methods A total of 100 elderly patients with non-melanoma skin cancer and precancerous lesions on their head and face treated in our hospital from January 2015 to December 2018 were included in this study.They were divided randomly into control groups(50 cases)and the observation group(50 cases).The control group was treated with ALA-PDT,and the observation group was done with ALA-PDT combined with cryotherapy.After 3 months of treatment,the short-term efficacy,patients’satisfaction,adverse reactions and quality of life of the two groups were observed and compared each other.Results After 3 months of treatment,the total clinical effective rate of the observation group(88.00%)was higher than that of the control group(74.00%)(P<0.05);the treatment satisfaction of the observation group was also higher than that of the control group(96.00%vs 80.00%,P<0.05);the total incidence of adverse reactions in the observation group was lower than that in the control group(12.00%vs28.00%,P<0.05);the SF-36 score of the observation group was(70.52±10.13)greater than that of the control group was(59.42±8.17)(P<0.05).Conclusion ALA-PDT combined with cryotherapy in the treatment of non melanoma skin cancer and precancerous lesions on the head and face of the elderly has significant short-term effect,high treatment satisfaction,less adverse reactions,and more obvious improvement in quality of life,which has certain clinical application value.
