阿司匹林联合低分子肝素钙治疗血栓前状态致早期习惯性流产的临床疗效分析

Clinical Analysis of Aspirin Combined with Low Molecular Weight Heparin Calcium in the Treatment of Early Habitual Abortion Caused by Prethrombus Status

目的分析阿司匹林联合低分子肝素钙治疗血栓前状态致早期习惯性流产的临床疗效。方法选取2017年1月2019年7月合肥市第二人民医院收治的血栓前状态致早期习惯性流产患者64例随机分为对照组和观察组2组,各32例。对照组给予低分子肝素钙治疗,观察组在对照组的基础上联合阿司匹林治疗,比较2组患者治疗后的凝血功能、妊娠结局和并发症情况等指标。结果2组患者治疗前D-二聚体(D-D)、凝血酶时间(TT)、纤溶酶原激活物抑制物-1(PAI-1)比较,差异无统计学意义(P>0.05),治疗后患者D-D、PAI-1指标低于治疗前,TT高于治疗前,差异有统计学意义(P<0.05);观察组患者足月产率及活产率高于对照组,流产率低于对照组,差异有统计学意义(P<0.05);观察组胎儿生长受限、妊娠期高血压、妊娠期糖尿病、羊水过多发生率均低于对照组,差异有统计学意义(P<0.05)。结论在血栓前状态致早期习惯性流产患者的治疗中,阿司匹林联合低分子肝素钙能够改善患者的凝血功能,促进病情恢复,保护母婴安全,同时预防多种妊娠并发症的发生,值得在临床上推广应用。

Objective To analyze the clinical efficacy of aspirin combined with low molecular weight heparin calcium in the treatment of early habitual abortion caused by prethrombotic state.Methods RandomLy divide 64 cases of early habitual abortion caused by prethrombotic state admitted to the Second People's Hospital of Hefei City in January 2017 and July 2019 into a control group and an observation group,with 32 cases in each group.The control group was given low-molecular-weight heparin calcium,and the observation group was combined with aspirin treatment on the basis of the control group to compare the blood coagulation function,pregnancy outcome,and complications of the two groups after treatment.Results There was no significant difference in D-dimer(DD),thrombin time(TT),and plasminogen activator inhibitor-1(PAI-1)before treatment between the two groups of patients(P>0.05).After treatment,the DD and PAI-1 indexes were lower than before treatment,and TT was higher than before treatment(P<0.05);the full-term birth rate and live birth rate of the observation group were higher than the control group,and the abortion rate was lower than the control group(P<0.05);the incidence of fetal growth restriction,pregnancy-induced hypertension,gestational diabetes,and polyhydramnios in the observation group were lower than those in the control group(P<0.05).Conclusion In the treatment of patients with early habitual abortion caused by prethrombotic state,aspirin combined with low molecular weight heparin calcium can improve the coagulation function of patients,while promoting the recovery of the disease and protecting the safety of mother and child,it can also prevent the occurrence of multiple pregnancy complications,It is worthy of clinical application.
