不同类型急性白血病患者血清IFN-γ、TGF-β、IL-6和IL-17水平变化及其预后判定分析 被引量:16

Analysis of the changes of serum IFN-γ,TGF-β,IL-6and IL-17levels in patients with different types of acute leukemia and their prognosis

摘要 目的探讨不同类型急性白血病患者血清干扰素-γ(IFN-γ)、转化生长因子-β(TGF-β)、白介素-6(IL-6)和白介素-17(IL-17)水平变化及其预后。方法选取我院2015年6月至2017年6月接诊的100例急性白血病患者进行研究,根据疾病类型将其均分为急性淋巴细胞白血病(ALL)组和急性髓细胞白血病(AML)组,此外,选择同期健康体检的患者100例作为对照组。对三组患者治疗后的临床疗效,治疗前后的血清IFN-γ、TGF-β、IL-6和IL-17水平及其预后进行比较分析。结果ALL组和AML组治疗总有效率比较差异无统计学意义(χ2=0.0603,P=0.8061)。治疗前,ALL组和AML组的IFN-γ水平明显低于对照组,TGF-β、IL-6和IL-17水平明显高于对照组,差异有统计学意义(P<0.05)。ALL组血清IFN-γ、IL-6水平明显高于AML组(P<0.05),但两组患者的血清TGF-β、IL-17水平比较差异无统计学意义(P>0.05)。ALL组显效患者血清IFN-γ水平高于无效患者(t=8.6250,P=0.0000),TGF-β水平低于无效患者(t=4.5539,P=0.0000),IL-6水平低于无效患者(t=12.9912,P=0.0000),IL-17水平低于无效患者(t=10.0341,P=0.0000)。AML组显效患者血清IFN-γ水平高于无效患者(t=27.4937,P=0.0000),TGF-β水平低于无效患者(t=4.6727,P=0.0000),IL-6水平低于无效患者(t=5.5393,P=0.0000),IL-17水平均低于无效患者(t=11.7118,P=0.0000)。结论急性白血病与血清IFN-γ、TGF-β、IL-6和IL-17水平密切相关,可把患者血清水平作为临床诊断急性白血病、评价预后的指标。 Objective To investigate the changes of serum interferon-γ(IFN-γ),transforming growth factor-β(TGF-β),interleukin-6(IL-6)and interleukin-17(IL-17)in different types of acute leukemia and their prognosis.Methods 100cases of acute leukemia patients received from June 2015to June 2017in our hospital were studied.According to the type of disease,they were divided into acute lymphoblastic leukemia(ALL)group and acute myeloid leukemia(AML)group.In addition,100healthy persons in the same period were selected as control group.The clinical efficacy of three groups of patients after treatment,and the levels of serum IFN-gamma,TGF-beta,IL-6and IL-17before and after treatment and their prognosis were compared and analyzed.Results There was no significant difference in the total effective rate between ALL group and AML group(χ2=0.0603,P=0.8061).Before treatment,the levels of IFN-gamma in ALL group and AML group were significantly lower than those in control group,and the levels of TGF-beta,IL-6and IL-17were significantly higher than those in control group(P<0.05).Serum levels of IFN-gamma and IL-6in ALL group were significantly higher than those in AML group(P<0.05),but there was no significant difference in serum levels of TGF-beta and IL-17between the two groups(P>0.05).The level of serum IFN-gamma in ALL group was higher than that in invalid patients(t=8.6250,P=0.0000),the level of TGF-beta was lower than that in invalid patients(t=4.5539,P=0.0000),the level of IL-6was lower than that in invalid patients(t=12.9912,P=0.0000),and the level of IL-17was lower than that in invalid patients(t=10.0341,P=0.0000).The serum levels of IFN-gamma in AML group were higher than those in invalid patients(t=27.4937,P=0.0000),TGF-beta was lower than those in invalid patients(t=4.6727,P=0.0000),IL-6was lower than those in invalid patients(t=5.5393,P=0.0000),and IL-17was lower than those in invalid patients(t=11.7118,P=0.0000).Conclusion Acute leukemia is closely related to serum levels of IFN-gamma,TGF-beta,IL-6and IL-17.Serum level of patients can be used as an index for clinical diagnosis and prognosis evaluation of acute leukemia.
