高血压性腔隙性脑梗死患者血压变异性与微出血的相关性分析 被引量:14

Analysis of correlation between blood pressure variability and microbleeds in patients with hypertensive lacunar cerebral infarction

摘要 目的研究高血压性腔隙性脑梗死(LI)患者血压变异性与发生脑微出血的相关性。方法经头颅MRI证实患有LI的高血压患者82例,其中经头颅SWI检查发现并发脑微出血(CMBs)患者47例,无CMBs患者(对照组)35例。记录患者的年龄、性别等一般资料,所有患者均接受24h动态血压监测,比较两组间24h平均收缩压(24hSBP)、24h平均舒张压(24hDBP)、24h收缩压变异性(24hSBPV)、24h舒张压变异性(24hDBPV)差异,分析CMBs数量与24hSBP、24hDBP、24hSBPV、24hDBPV的相关性。结果CMBs组24hSBPV高于对照组,差异有统计学意义(P<0.05),两组间24hSBP、24hDBP、24hDBPV无显著差异(P>0.05);Pearson相关性分析显示,CMBs组CMBs数量与24hSBPV、24hDBPV呈显著正相关(P<0.05)。结论高血压性LI患者血压变异性升高与并发脑微出血相关,血压变异性升高可能是高血压性LI患者并发微出血的危险因素。 Objective To study the correlation between blood pressure variability(BPV)and cerebral microbleeds(CMBs)in patients with lacunar infarction(LI).Methods Totally 82 hypertensive patients were diagnosed LI through magnetic resonance imaging(MRI).Of whom 47 patients were found CMBs and 35 patients were not found CMBs through susceptibility-weighted imaging(SWI).The general information such as age and gender was recorded.All patients received 24-hour ambulatory blood pressure monitoring.The 24-hour mean systolic blood pressure(24hSBP),24-hour mean diastolic blood pressure(24hDBP),24-hour systolic blood pressure variability(24hSBPV)and 24-hour diastolic blood pressure variability(24hDBPV)were compared between the CMBs group and non-CMBs group.The correlation between the CMBs quantity and 24hSBP,24hDBP,24hSBPV,24hDBPV were analyzed.Results The 24hSBPV of CMBs group was higher than that of control group(P<0.05).There was no significant difference in 24hSBP,24hDBP and 24hDBPV between the two groups(P>0.05);Pearson correlation analysis showed that the quantity of CMBs was correlated with 24hSBPV and 24hDBPV(P<0.05).Conclusion The increase of BPV is related to CMBs in patients with hypertensive LI.
