居家药学服务对老年缺血性脑卒中患者二级预防的作用

Effect of home pharmaceutical service on secondary prevention medication among elderly patients with ischemic stroke

目的研究居家药学服务对老年缺血性脑卒中患者二级预防的作用。方法选择居家治疗的老年缺血性脑卒中患者136例,随机分组:对照组69例,无居家药学服务干预措施;干预组67例,持续提供居家药学服务。运用欧洲药学监护联盟(PCNE)分类系统8.03版对药物相关问题进行分类。结果6个月后随访,干预组与对照组相比,用药依从性、每日口服用药总片数、口服保健品片数、药物疗效及药物安全性相关用药问题个数差异均有统计学意义(P<0.05)。干预组卒中复发率显著低于对照组(P<0.05)。结论居家药学服务在一定程度上规范了老年缺血性脑卒中患者的二级预防。

Objective To determine the effect of home pharmarceutical service on secondary prevention medication among elderly patients with ischemic stroke.Methods Totally 136 elderly patients with ischemic stroke were selected and randomly divided into a control group(69 patients)with general data alone,and a treatment group(67 patients)with continuous home pharmaceutical service.Pharmaceutical Care Network Europe(PCNE)Classification version 8.03 was used to classify drug-related problems.Results Six months later,compared with the control group,the patients in the treatment group showed significant difference in such indicators as medication compliance,total number of daily oral medication tablets,amount of oral health product,drug efficacy,and number of medication problems related to drug safety(P<0.05).The recurrence rate of stroke in the treatment group was significantly lower than that in the control group(P<0.05).Conclusion Home pharmaceutical service helps secondary prevention in elderly patients with ischemic stroke.
