床旁超声及CT平扫在急症主动脉夹层动脉瘤诊断中的应用

Application of Bedside Ultrasound and CT in the Diagnosis of Cute Aortic Dissection Aneurysm

目的探讨床旁超声及CT检查在急症主动脉夹层动脉瘤诊断中的应用。方法选取本院2018年6月至2019年2月收治的60例急症主动脉夹层动脉瘤患者。收集患者临床及影像学资料。观察分析床旁超声检查和CT检查主动脉夹层动脉瘤的影像学表现,以术后病理诊断为金标准,对比床旁超声检查和CT检查的准确性、敏感性、特异性。结果床旁超声检查对主动脉夹层动脉瘤的诊断灵敏性、特异性和准确性分别为68.33%、71.67%、73.33%,CT检查对主动脉夹层动脉瘤的诊断灵敏性、特异性和准确性分别为88.33%、90.00%、91.67%。CT检查对主动脉夹层动脉瘤的诊断灵敏性、特异性和准确性明显高于床旁超声检查,两者比较差异有统计学意义(P<0.05)。结论床旁超声和CT检查均可有效的显示主动脉夹层动脉瘤的影像学特点,但CT检查鉴别诊断主动脉夹层动脉瘤的能力优于床旁超声,临床可根据患者个人情况来选择。

Objective To explore the application of bedside ultrasound and CT in the diagnosis of cute aortic dissection aneurysm.Methods 60 patients with acute aortic dissection aneurysm admitted to our hospital from June 2018 to February 2019 were enrolled.Clinical and imaging data of patients were collected.To observe the imaging findings of aortic dissection aneurysm in bedside ultrasound and CT.Postoperative pathological diagnosis was used as the gold standard to compare the accuracy,sensitivity and specificity between bedside ultrasound and CT examination.Results The sensitivity,specificity,and accuracy of bedside ultrasound for aortic dissection aneurysms were 68.33%,71.67%,and 73.33%,respectively.The diagnostic sensitivity,specificity,and accuracy of CT examination for aortic dissection aneurysms were 88.33%,90.00%,and 91.67%.The sensitivity,specificity and accuracy of CT examination for aortic dissection aneurysm were significantly higher than those of bedside ultrasound.The difference was statistically significant(P<0.05).Conclusion Bedside ultrasound and CT examination can effectively show the imaging features of aortic dissection aneurysms,but the ability of CT to identify and diagnose aortic dissection aneurysms is superior to that of bedside ultrasound.In clinic,they can be selected according to the individual's condition.
