消癥化积方灌肠联合双歧杆菌对结直肠癌患者术后肠道微生态及黏膜免疫功能的影响

Effect of Xiaozheng Huaji recipe enema combined with Bifidobacterium on intestinal microecology and mucosal immune function in patients with colorectal cancer after surgery

目的探究消癥化积方灌肠联合双歧杆菌治疗对结直肠癌患者术后肠道微生态及黏膜免疫功能的影响。方法按照治疗方法的不同将218例结直肠癌患者分为观察组与对照组,每组109例。对照组患者于术后采用双歧杆菌活菌胶囊进行常规治疗,观察组在对照组的基础上加用消癥化积方灌肠治疗。术后1周,比较两组患者的临床疗效、中医证候积分、肠道菌群数量、免疫功能、血清炎症因子水平及术后感染相关并发症的发生情况。结果治疗后,观察组患者的总有效率明显高于对照组,中医证候积分明显低于对照组(P﹤0.01)。治疗后,两组患者的乳酸杆菌、粪肠球菌、双歧杆菌数量均较本组治疗前降低,大肠杆菌数量均较本组治疗前升高(P﹤0.05);治疗后,观察组患者的乳酸杆菌、粪肠球菌、双歧杆菌数量均明显高于对照组,大肠杆菌数量明显低于对照组(P﹤0.01)。治疗后,观察组患者的超敏C-反应蛋白(Hs-CRP)、肿瘤坏死因子α(TNF-α)水平以及术后感染相关并发症的总发生率均明显低于对照组(P﹤0.01)。结论消癥化积方灌肠联合双歧杆菌有利于改善结直肠癌患者术后肠道微生态及黏膜免疫功能,维持肠道菌群稳定,减少术后炎症反应及感染并发症。

Objective To investigate the effects of Xiaozheng Huaji recipe enema combined with Bifidobacterium on the intestinal microecology and mucosal immune function in patients with colorectal cancer after surgery.Method A total of 218 patients with colorectal cancer were enrolled and divided into study group and control group according to respective therapies administered,with 109 cases in each.The control group was treated with Bifidobacterium live bacteria capsules,meanwhile the study group was administered with Xiaozheng Huaji recipe as enema in addition to the treatment for the control group.One week after surgery,the clinical efficacy,traditional Chinese medicine syndrome scores,intestinal microbiota,immune function,serum inflammatory factors and postoperative complications of infection were observed in both groups.Result After treatment,the overall response rate in study group was significantly higher than that of the control group,the traditional Chinese medicine syndrome score decreased significantly in study group(P<0.01).Besides,the number of Lactobacillus,Enterococcus faecalis,Bifidobacterium decreased,while the number of Escherichia coli increased in both groups after treatment(P<0.05),with greater change observed in study group versus control group(P<0.01).The high-sensitivity C-reactive protein(Hs-CRP)and tumor necrosis factor-α(TNF-α)levels were decreased in study group than in control group,and the incidence of overall incidence rate of complications of infections in study group was evidently lower in study group in comparison with control group.Conclusion Therapy of Xiaozheng Huaji recipe enema combined with Bifidobacterium is beneficial to improving intestinal microecology and mucosal immune function in patients with colorectal cancer after surgery,maintaining intestinal microbiota stability,and thus reducing postoperative inflammatory response and complication of infections.
