经尿道输尿管镜下钬激光碎石术对输尿管结石患者肾功能及失血量的影响

Effect of Holmium Laser Lithotripsy under Transurethral Ureteroscope on Renal Function and Blood Loss in Patients with Ureterolithiasis

目的探讨经尿道输尿管镜下钬激光碎石术对输尿管结石患者肾功能及失血量的影响。方法回顾性分析2017年3月至2019年5月于福建省漳州市中医院接受手术治疗的73例输尿管结石患者的临床资料,根据治疗方式不同分为试验组(37例,采用经尿道输尿管镜下钬激光碎石术)与对照组(36例,采用气压弹道碎石术),比较两组的结石完全清除率、手术时间、术中失血量、住院时间与肾功能指标[血肌酐(SCr)、血尿素氮(BUN)]水平。结果试验组结石完全清除率高于对照组,差异有统计学意义(P<0.05);试验组手术时间、住院时间均短于对照组,术中失血量少于对照组,差异均有统计学意义(P<0.05);术后,两组SCr、BUN水平均降低,且试验组低于对照组,差异均有统计学意义(P<0.05)。结论经尿道输尿管镜下钬激光碎石术具有手术时间短、术中失血少、恢复快的优点,能够清除输尿管结石,改善患者的肾功能。

Objective To analyze the effect of holmium laser lithotripsy under transurethral ureteroscopy on patients with ureteral calculi in order to provide basis for clinical application.Methods A retrospective analysis of clinical data of 73 patients with ureteral calculi who underwent surgical treatment in our hospital from March 2017 to May 2019.According to the differences in treatment methods,the above-mentioned enrollees were divided into experimental group(37 cases,holmium laser lithotripsy under transurethral ureteroscopy)and control group(36 cases,pneumatic ballistic lithotripsy),compared the complete stone removal rate,operation time,blood loss during operation,hospitalization time,and renal function indicators in the two groups[serum creatinine(SCr),blood urea?nitrogen(BUN)]levels.Results The complete stone removal rate in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time and hospital stay in the experimental group were shorter than the control group,and the blood loss during the operation was less than that in the control group,and the differences were statistically significant(P<0.05).After surgery,the levels of SCr and BUN in the two groups decreased,and the levels of SCr and BUN in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions Holmium laser lithotripsy under transurethral ureteroscopy has the advantages of short time,less blood loss,and quick recovery.It can effectively clear ureteral stones and improve renal function in patients.
