通过病机分析及医案举例的方法,介绍梁如镜主任医师治疗糖尿病眼底出血的经验。梁老认为糖尿病患者多郁血虚损,而且久病入络,所以眼中络脉必有瘀阻郁结不通。糖尿病眼底出血其基本病机为本瘀标热:当肝失疏泄、少阳气郁化火,循经上迫时,气与血并,迫血妄行而发病。针对其本瘀标热的病机,应该从郁、瘀两方面论治。治疗上使用:清热解郁、消瘀通络为主要方法,泻其郁热、通其血道,使热清络通,血归其经而行则血可自止。方药选择上:早期选用小柴胡汤合桃核承气汤加减;中晚期选用小柴胡汤合补阳还五汤加减;当瘀血已成干血难消者,则宜加用水蛭并且重用。

Through the analysis of pathogenesis and examples of medical records,this paper introduces Dr Liang Rujing's experience in the treatment of diabetic fundus hemorrhage.Dr Liang from stagnant blood deficiency,long-term disease into collaterals,blood stasis standard heat argument;basic pathogenesis of diabetic fundus hemorrhage is fever due to stasis:when the liver failure,less Yang Qi Yu Huhuo,along the meridians when forced,Qi and blood and forced blood to act in vain and morbidity treatment,using Jieyu Qingre,Xiaoyu Tongluo as the main treatment methods;Xie its yure,its blood,so that the heat Qingluo Tong,blood to its meridian and blood can be self-control.Prescription selection:early selection of Xiao Chaihu decoction combined with Taohe Chengqi decoction;in the middle and late stage,Xiao Chaihu decoction combined with Buyang Huanwu decoction is used to add and subtract;when blood stasis has become dry blood is difficult to eliminate,it is appropriate to add leeches and reuse.
