摘要 目的观察超声引导下勾刀治疗成人扳机指(trigger finger,TF)的临床疗效。方法80例成人扳机指患者随机分为治疗组和对照组,治疗组在超声引导下行勾刀治疗,对照组在传统盲视条件下行勾刀治疗,所有患者均定期复查随访3个月,评价疗效。并对所有患者手指活动过程中的疼痛评分进行记录和统计,具体记录时段为:治疗前、治疗后即刻、治疗后1周及治疗后3个月,疼痛评分依据疼痛视觉模拟评分法(VAS)进行分析统计。结果80例患者均获得随访3个月,治疗组患者在接受治疗后即刻、治疗后1周及治疗后3个月这3个时间点的手指活动时VAS评分均小于对照组,根据对患者的后续随访和检查,治疗组患者在接受超声引导下勾刀治疗3个月后的总治愈率达85.00%,1次操作治愈率达90.00%,对照组的总治愈率达52.50%,1次操作治愈率达62.50%,治疗组患者的总治愈率和一次操作治愈率均明显高于对照组。结论超声引导下勾刀治疗成人扳机指能准确定位病灶,提高安全性和治疗精度。 Objective To observe the clinical efficacy of ultrasound-guided hook knife in the treatment of adult trigger finger(TF).Methods 80 patients with adult trigger finger were randomly divided into treatment group and control group.The treatment group was treated with ultrasound-guided hook knife.The control group was treated with hook knife under the traditional blind condition.All patients were followed up for 3 months.The efficacy was evaluated.Pain scores were recorded and counted during finger activity in all patients.The specific recording period was:before treatment,immediately after treatment,one week after treatment,and three months after treatment.Pain scores were analyzed based on pain visual analogue scale(VAS).Results All the 80 patients were followed up for 3 months.The VAS scores of the patients in the treatment group were lower than those in the control group immediately after receiving the treatment,one week after treatment and three months after treatment,and the difference is statistically significant(P<0.05).According to the follow-up and examination of the patients,the total cure rate of the treatment group after treatment with ultrasound-guided hook knife for 3 months reached 85.00%,and the cure rate of one operation was 90.00%.The total cure rate of the control group was 52.50%,and the cure rate of one operation was 62.50%.The total cure rate and the cure rate of one operation in the treatment group were significantly higher than those in the control group(P<0.05).Conclusion Ultrasound-guided hook knife can accurately locate the lesions and improve the safety and treatment accuracy.
