冠脉CT成像技术在冠心病诊断中的应用价值

Practical value of coronary CT imaging in the diagnosis of coronary heart disease

目的探讨冠状动脉(冠脉)CT成像技术对冠心病诊断的实际价值。方法选取96例疑似冠心病患者作为研究对象,均行冠脉CT成像检查及冠脉造影检查。以冠脉造影检查结果为金标准,观察两种检查方法对冠脉狭窄节段数及冠脉狭窄程度检出情况。结果96例患者经冠脉造影检出狭窄节段共计858段,其中右冠脉狭窄272段,左主干支狭窄82段,左前降支狭窄296段,左回旋支狭窄208段;冠脉CT成像检出狭窄节段共计833段,其中右冠脉狭窄259段,左主干支狭窄79段,左前降支狭窄292段,左回旋支狭窄203段;以冠脉造影为金标准,冠脉CT成像对狭窄节段的检出符合率为97.09%(833/858)。96例患者应用冠脉造影检查发现狭窄858段,轻度狭窄385段,中度狭窄362段,重度狭窄111段;应用螺旋CT成像技术检查发现狭窄833段,轻度狭窄375段,中度狭窄354段,重度狭窄104段。冠脉CT成像对轻度狭窄、中度狭窄、重度狭窄的诊断符合率分别为97.40%、97.79%、93.69%。结论冠脉CT成像技术具有操作简单、无创、安全性高的优点,对冠心病的诊断准确性高,是冠心病诊断的首选方法,对冠心病的早期诊断、早期治疗具有重要意义。

Objective To discuss the practical value of CT coronary angiography in the diagnosis of coronary heart disease.Methods There were 96 patients with suspected coronary heart disease were selected as the study subjects,all of them were examined by coronary CT imaging and coronary angiography.With the results of coronary angiography as the gold standard,the detection status of the number of coronary stenosis segments and the degree of coronary stenosis by two methods were observed and compared.Results 858 stenosed segments were found by coronary angiography,including 272 right coronary arteries,82 left main arteries,296 left anterior descending arteries and 208 left circumflex arteries in 96 patients.833 stenosed segments were found by coronary CT imaging,including 259 right coronary arteries,79 left main arteries,292 left anterior descending arteries and 203 left circumflex arteries.With the results of coronary angiography as gold standard,the coincidence rate of detection of stenosis segment by coronary CT was 97.09%(833/858).858 stenosed segments were found by coronary angiography,including 385 segments of mild stenosis,362 segments of moderate stenosis and 111 segments of severe stenosis in 96 patients.833 stenosed segments were found by coronary CT imaging,including 375 segments of mild stenosis,354 segments of moderate stenosis and 104 segments of severe stenosis.The diagnostic accuracy of coronary CT in mild stenosis,moderate stenosis and severe stenosis were 97.40%,97.79%and 93.69%,respectively.Conclusion Coronary CT imaging technology has the advantages of simple operation,non-invasiveness,and high safety.It has high diagnostic accuracy for coronary heart disease and is the preferred method for the diagnosis of coronary heart disease.It is of great significance for the early diagnosis and treatment of coronary heart disease.
