体感音乐治疗联合四肢联动训练对痉挛期脑卒中患者运动功能疗效观察

Effect of vibroacoustic therapy combined with limb joint training on motor function in patients with stroke in the spastic stage

目的观察体感音乐治疗联合四肢联动训练对痉挛期脑卒中患者运动功能的疗效。方法将某院康复中心60例痉挛期脑卒中住院患者作为研究对象,随机数表法分为两组,对照组30例给予常规康复训练,观察组30例在对照组基础上给予体感音乐治疗联合四肢联动训练;分别在治疗前、治疗1个月后、治疗3个月后采用Fugl-Meyer运动功能评定法(FMA)、Berg平衡量表评定法(BBS)、改良Ashworth评定法、改良Barthel指数评定法(MBI)进行评定。结果两组患者治疗前一般情况比较差异无统计学意义(P>0.05);两组患者治疗1个月后、治疗3个月后FMA评分、BBS评分、改良Ashworth评分、MBI评分与治疗前相比均有所提高;与对照组相比,观察组治疗1个月后、治疗3个月后FMA评分、BBS评分、改良Ashworth评分、MBI评分均提高,差异有统计学意义(P<0.05)。结论体感音乐治疗联合四肢联动训练对痉挛期脑卒中患者运动功能改善明显优于常规康复训练,更有助于缓解痉挛肌的肌张力和改善平衡功能,提高患者日常生活自理能力和生活质量。

Objective To observe the effect of vibroacoustic therapy combined with limb joint training on motor function in patients with stroke in the spastic stage.Methods A total of 60 hospitalized patients with stroke in the spastic stage in a rehabilitation center of a sanatorium were taken as the research objects.They were randomly divided into two groups by random number table method.30 patients in the control group were given routine rehabilitation training,and 30 patients in the observation group were given vibroacoustic therapy combined with limb joint training on the basis of the control group.Fugl-meyer motor function assessment method(FMA),Berg balance scale assessment method(BBS),improved Ashworth assessment method and improved Barthel index assessment method(MBI)were adopted for assessment before treatment,1 month after treatment and 3 months after treatment.Results There was no significant difference in the general conditions between the two groups before treatment(P>0.05).FMA score,BBS score,improved Ashworth score and MBI score of patients in both groups were improved after 1 month and 3 months of treatment.Compared with the control group,FMA score,BBS score,improved Ashworth score and MBI score were all improved in the observation group after 1 month and 3 months of treatment,and the difference was statistically significant(P<0.05).Conclusion Vibroacoustic therapy combined with limb joint training can significantly improve motor function of patients with stroke in the spastic stage,which is better than routine rehabilitation training,and more helpful to relieve muscle tension of spastic muscles,improve balance function,and thus improve patients'ability to take care of themselves and quality of life in daily life.
