摘要
目的比较研究高分辨率磁共振血管壁成像(high-resolution magnetic resonance imaging,HR-MRI)与数字减影血管造影(DSA)对颅内动脉狭窄和闭塞的诊断。方法选择2016年10月至2019年1月在某院就诊的颅内动脉狭窄患者72例,分别采用HR-MRI和DSA对其进行颅内动脉狭窄的诊断,比较两种方式对不同程度颅内狭窄的诊断结果、不同形态斑块的诊断结果以及患者的满意度。结果HR-MRI和DSA都可以有效诊断不同程度的颅内动脉狭窄,两者在重度狭窄和闭塞上诊断结果保持一致,HR-MRI在轻度狭窄和中度狭窄的诊断上稍低于DSA,但是两者差异无统计学意义(P>0.05);高分辨磁共振血管壁成像(HR-MRI)在斑块形态诊断上准确率优于DSA(P<0.05)。结论HR-MRI和DSA对颅内动脉狭窄、闭塞患者的诊断结果都相对可靠,但是高分辨磁共振血管壁成像(HR-MRI)在斑块形态诊断以及患者满意度上具有明显优势,结果可为患者情况评估和治疗提供依据。
Objective To compare high resolution magnetic resonance imaging(HR-MRI)and DSA in the diagnosis of intracranial arterial stenosis and occlusion.Methods A total of 72 patients with intracranial artery stenosis admitted to a hospital from October 2016 to January 2019 were selected and diagnosed with intracranial artery stenosis by HR-MRI and DSA,respectively.The results of diagnosis of intracranial stenosis with different degrees,diagnosis of plaques with different shapes and patient satisfaction were compared between the two methods.Results High resolution magnetic resonance imaging(HR-MRI)and DSA can both effectively diagnose intracranial artery stenosis of different degrees,and the diagnosis results of severe stenosis and occlusion are consistent.HR-MRI is slightly lower than DSA in the diagnosis of mild stenosis and moderate stenosis,but the difference between them was not statistically significant(P>0.05).HR-MRI was more accurate than DSA in the diagnosis of plaque morphology(P<0.05).Conclusion High-resolution magnetic resonance imaging(HR-MRI)and DSA are relatively reliable in the diagnosis of patients with intracranial artery stenosis and occlusion,but HR-MRI has obvious advantages in the diagnosis of plaque morphology and patient satisfaction,and the results can provide a basis for the evaluation and treatment of patients.
