老年骨盆骨折的手术治疗

Surgical treatment of geriatric pelvic fractures

目的探讨老年骨盆骨折手术治疗的临床效果。方法回顾分析2015年1月至2019年12月北京积水潭医院创伤骨科行手术治疗的19例老年骨盆骨折患者的临床资料。其中男8例,女11例;平均年龄72岁(65~89岁)。依据不同骨折分型标准分为Tile B型17例,C型2例;FFPⅡ型2例,Ⅲ型15例,Ⅳ型2例。手术采用骨盆前后环固定12例,单独后环固定5例,单独前环固定2例。依据患者疼痛程度、步态及生活自理能力评定治疗的功能结果。结果骨折复位优14例,良3例,可2例。平均随访时间为21个月(4~53个月)。病死1例;功能结果优12例,良6例。结论手术治疗老年骨盆骨折可以获得令人满意的临床效果。

Objective To investigate the clinical results of surgical treatment for geriatric pelvic fractures.Methods Clinical data of 19 cases of geriatric pelvic fractures treated by surgical operation in Department of Orthopedic Trauma,Beijing Jishuitan Hospital between January 2015 and December 2019 were reviewed.There were 8 males and 11 females with an average age of 72 years(65-89 years).The fracture types included 17 cases in type B and 2 cases in type C according to Tile classification,and 2 cases in typeⅡ,15 cases in typeⅢand 2 cases in typeⅣaccording to the fragility fracture of pelvis(FFP)classification.Both the pelvic anterior and posterior rings were fixed in 12 cases,only posterior rings were fixed in 5 cases,and only anterior rings were fixed in 2 cases.The clinical results were graded by the evaluation of patient's pain level,gait and self-care ability.Results The reduction were excellent in 14 cases,good in 3 cases and fair in 2 cases.All the 19 cases obtained follow-up for an average time of 21 months(4-53 months).One patient died within the first year after surgery.The functional results were excellent in 12 cases and good in 6 cases.Conclusion Surgical treatment is an effective method for treatment of the geriatric pelvic fractures.
