357例老年椎体压缩性骨折手术患者临床特征分析

Analysis of clinical features of 357 elder patients with vertebral compression fracture

目的通过分析近5年某院50岁以上压缩性骨折手术患者的基本情况和临床特征,为本地区老年患者脊柱骨折的防治提供参考依据。方法从医院电子病案系统收集2014年1月至2018年12月胸腰椎骨折手术患者资料,回顾性分析患者的性别、年龄、出院日期、骨折部位、骨折节段、手术方式、住院时间、住院总费用等。结果近5年胸腰椎骨折手术患者共357例,年龄最大96岁,平均年龄(73.59±9.73)岁,每年女性手术患者人数均多于男性,差异有统计学意义(P<0.05)。骨折部位差异无统计学意义,手术方法发生明显变化(P<0.05)。单节段骨折为主,骨折好发节段在T11-L2段,占64.4%。结论该院近5年收治的符合纳入标准的老年胸腰椎骨折手术患者中,女性多于男性,60岁以上高发,T12和L1节段骨折所占比例最高。应针对以上临床特征积极防治骨质疏松和脊柱骨折。

Objective To analyze the basic situation and clinical characteristics of patients over 50 years old undergoing compression fracture surgery in our hospital in the past five years,so as to provide a reference for the prevention and treatment of spinal fracture in elderly patients in this area.Methods The data of patients who received spinal fractures surgery in the hospital from Jan.2014 to Dec.2018 were collected through hospital medical record system.Clinical parameters such as gender,age,discharge date,fracture site,fracture segment,surgical procedure,length of stay,and the total cost of stay were analyzed.Results Totally 357 patients with the maximum age of 96 years old and the average age of 73.59±9.73 years old were included in the retrospective survey.The number of female surgical patients was higher than that of male patients every year,with statistically significant difference(P<0.05).There was no difference between the fracture sites;however,there was significant changes between surgical methods(P<0.05).Single segment fractures were dominant,and the most common segment was T 11-L 2 segment,accounting for 64.4%.Conclusion The incidence of spinal fractures among elderly patients is higher in female patients than that in male,especially with a higher incidence in over 60 years old patients.The highest proportions are T 12 and L 1 segment fractures.Active prevention and treatment should be carried out to reduce the occurrence of spinal fractures.
