功能性鼻内镜手术及轮廓化鼻内镜手术治疗对难治性鼻窦炎患者临床疗效及预后的影响比较

Comparison of Clinical Effects and Prognosis of Functional Endoscopic Sinus Surgery and Contoured Endoscopic Sinus Surgery on Refractory Sinusitis Patients

目的探讨功能性鼻内镜手术及轮廓化鼻内镜手术治疗对难治性鼻窦炎患者临床疗效及预后的影响。方法将偃师市人民医院在2017年9月-2019年2月期间收治的60例难治性鼻窦炎患者随机分为对照组和观察组,每组30例,对照组进行功能性鼻内镜手术治疗,观察组进行轮廓化鼻内镜手术治疗。通过比较两组患者治疗前后的VAS评分、内镜Lund-Kennedy评分以及有效率来评估临床疗效和预后影响。结果干预前两组患者的VAS评分差异无统计学意义(P>0.05),经过干预,两组患者VAS评分均有所下降,观察组下降程度明显高于对照组,差异有统计学意义(P<0.05)。且两组患者干预前Lund-Kennedy评分差异无统计学意义(P>0.05),经过干预,两组患者Lund-Kennedy评分均有所下降,观察组明显低于对照组,差异有统计学意义(P<0.05)。此外,观察组患者疗效高达83.33%,明显高于对照组的66.67%,差异有统计学意义(P<0.05)。结论轮廓化鼻内镜手术能够减轻鼻窦炎患者的疼痛程度,具有很好的临床疗效。

Objective To investigate the effect of the clinical efficacy and prognosis of functional endoscopic sinus surgery and contoured endoscopic sinus surgery on patients with refractory sinusitis.Methods 60 patients with refractory nasosinusitis admitted to the hospital from September,2017 to February,2019 were randomly divided into control group and observation group,with 30 cases in each group.The control group received functional endoscopic surgery,and the observation group received contoured endoscopic surgery.The VAS score,endoscopic Lund-Kennedy score and effective rate of the two groups of patients before and after treatment were compared to evaluate the clinical efficacy and prognosis.Results There was no significant difference in VAS scores between the two groups before the intervention(P>0.05).After the intervention,the VAS scores of the two groups decreased,and the degree of decrease in the observation group was significantly higher than that in the control group,with significant difference(P<0.05).There was no significant difference in Lund-Kennedy scores between the two groups before the intervention(P>0.05).After intervention,Lund-Kennedy scores of the two groups decreased,and the observation group was significantly lower than the control group,with significant difference(P<0.05).In addition,the curative effect of the observation group was 83.33%,which was significantly higher than 66.67%of the control group,with statistically significant difference(P<0.05).Conclusion Contoured endoscopic sinus surgery can relieve the pain of patients with sinusitis,and has good clinical effect.
