摘要
目的探讨高频彩色多普勒超声诊断缺血性脑卒中患者颈动脉粥样硬化的价值。方法选取肇庆市中医院2018年6月-2019年6月期间收治的缺血性脑卒中患者130例作为病例组,另选取同时期在我院进行健康体检提示无任何疾病的130例为健康组,两组入选者均给予高频彩色多普勒超声检查,比较两组患者颈动脉内中膜厚度、颈动脉斑块位置及斑块内部回声、血管狭窄程度的情况。结果病例组的颈内动脉、颈外动脉及颈总动脉内中膜厚度均高于健康组,差异有统计学意义(P<0.05);两组颈总动脉、颈总动脉分叉处及颈内动脉起始部的斑块检出率,差异无统计学意义(P>0.05);病例组的低、等回声斑块明显高于健康组,差异有统计学意义(P<0.05);病例组血管狭窄程度高于健康组,差异有统计学意义(P<0.05)。结论应用高频彩色多普勒超声诊断缺血性脑卒中患者颈动脉粥样硬化的效果较好,可以检测出颈动脉内中膜厚度、斑块的位置、斑块的内部回声及血管狭窄程度,诊断结果具有客观性。
Objective To investigate the value of high frequency color Doppler ultrasonography in the diagnosis of carotid atherosclerosis in patients with ischemic stroke.Methods A total of 130 patients with ischemic stroke admitted to the hospital from June,to June,2019 were enrolled as the case group.130 patients who had no physical disease at the same time in the hospital were selected as the health group.High-frequency color Doppler ultrasonography was performed in the group.The carotid intima-media thickness,carotid plaque location,internal plaque echo and vascular stenosis were compared between the two groups.Results The medial thickness of the internal carotid artery,external carotid artery and common carotid artery in the case group were higher than those in the healthy group(P<0.05).There was no significant difference in the common carotid artery,common carotid bifurcation and internal artery between two groups(P>0.05).The low and equal echo plaques in the case group were significantly higher than those in the healthy group(P<0.05).The degree of vascular stenosis was higher in the group than in the healthy group,and the difference was statistically significant(P<0.05).Conclusion High-frequency color Doppler ultrasonography is effective in the diagnosis of carotid atherosclerosis in patients with ischemic stroke.It can detect carotid intima-media thickness,plaque location,internal echo of plaque and degree of vascular stenosis.The diagnosis results are objective.
