拓扑异构酶Ⅱα在粤东地区乳腺癌中的表达及其临床意义

Expression and clinical significance of TopoⅡαin breast cancer in eastern Guangdong

目的:探讨粤东地区乳腺癌中拓扑异构酶Ⅱα(TopoⅡα)的表达情况及其临床意义。方法:收集2015—2016年于汕头大学医学院附属肿瘤医院就诊的乳腺癌患者术后石蜡包埋组织424例,采用免疫组化方法检测乳腺癌组织中TopoⅡα的表达,分析其与临床病理特征的关系;利用Kaplan-Meier plotter分析TopoⅡα表达与乳腺癌患者预后的相关性。结果:免疫组化检测雌激素受体阴性、孕激素受体阴性及人类表皮生长因子受体2(HER2)阳性的乳腺癌患者的TopoⅡα分级更高(P<0.05);Ki-67指数、组织学级别与TopoⅡα分级呈正相关(P<0.001);HER2过表达型及三阴性乳腺癌TopoⅡα的Ⅱ~Ⅳ级比例更高(P<0.01);TopoⅡα与年龄、T分期、淋巴结是否转移无相关性(P>0.05);TopoⅡα表达越高,乳腺癌患者无复发生存率及总生存率越低。结论:粤东地区乳腺癌中,TopoⅡα的高表达与乳腺癌高增殖能力及高组织学级别呈正相关;TopoⅡα的高表达常提示不良预后。

Objective:To investigate the expression and clinical significance of TopoⅡαin breast cancer in eastern Guangdong.Methods:From 2015 to 2016,424 cases of breast cancer patients treated in the Affiliated Tumor Hospital of Shantou University Medical College were collected.The expression of TopoⅡαin breast cancer tissues was detected by immunohistochemistry,and the relationship between TopoⅡαand clinical pathological characteristics was analyzed.Kaplan-Meier plotter was used to analyze the correlation between TopoⅡαexpression and the prognosis of breast cancer patients.Results:Among the ER negative,PR negative and HER2 positive breast cancer patients,TopoⅡαgrade was higher(P<0.05);Ki-67 index and histological grade were positively correlated with TopoⅡαgrade(P<0.001);HER2 overexpression and three negative breast cancer TopoⅡαhave a higher proportion ofⅡ-Ⅳgrade(P<0.01);TopoⅡαhad no correlation with age,T stage and lymph node metastasis(P>0.05);The higher the expression of TopoⅡα,the lower the recurrence free survival rate and the overall survival rate of breast cancer patients.Conclusion:The high expression of TopoⅡαis positively correlated with the high proliferative ability and high histological grade of breast cancer in the east of Guangdong Province.
