射血分数保留的心衰患者血清NT-proBNP水平与心脏舒张功能的相关性

Correlation between serum NT-proBNP level and cardiac diastolic function in patients with heart failure with preserved ejection fraction

目的:探讨射血分数保留的心衰(HFpEF)患者血清N末端脑钠肽前体(NT-proBNP)水平与心脏舒张功能的相关性。方法:选择2017年1月~2017年12月于我院治疗的50例HFpEF患者为HFpEF组,另选择同期健康者50例作为健康对照组。检测比较两组血清NT-proBNP水平和心脏舒张功能指标,并分析血清NT-proBNP水平与心脏舒张功能的相关性。结果:与健康对照组比较,HFpEF组二尖瓣环舒张早期运动速度(Em)[(0.10±0.01)m/s比(0.07±0.02)m/s]、LVEF[(0.71±0.07)比(0.66±0.05)]均显著降低,血清NT-proBNP水平[(107.18±11.04)μg/L比(1366.32±15.75)μg/L]、E/Em[(8.12±1.44)比(14.21±4.32)]、左室心肌质量指数(LVMI)[(110.22±42.16)g/m^2比(138.46±34.11)g/m^2]、E减速时间(DT)[(192.36±52.68)s比(204.11±50.12)s]均显著升高(P均=0.001)。Pearson相关分析显示,血清NT-proBNP水平与E/Em、LVMI呈显著正相关(r=0.654,0.755),与LVEF呈显著负相关(r=-0.803),P均=0.001。结论:血清NT-proBNP水平与E/Em、LVMI、LVEF有显著相关性,因而,血清NT-proBNP水平的检测对HFpEF有一定的辅助诊断价值。

Objective:To explore correlation between serum level of N terminal pro brain natriuretic peptide(NT-proBNP)and cardiac diastolic function in patients with heart failure with preserved ejection fraction(HFpEF).Methods:A total of 50 HFpEF patients treated in our hospital from Jan 2017 to Dec 2017 were selected as HFpEF group.Another 50 healthy people were enrolled as healthy control group simultaneously.Serum NT-proBNP level and cardiac diastolic function indexes were measured and compared between two groups.Correlation between serum NT-proBNP level and cardiac diastolic function was analyzed.Results:Compared with healthy control group,there were significant reductions in peak early diastolic mitral annular velocity(Em)[(0.10±0.01)m/s vs.(0.07±0.02)m/s]and LVEF[(0.71±0.07)vs.(0.66±0.05)],and significant rise in serum NT-proBNP level[(107.18±11.04)μg/L vs.(1366.32±15.75)μg/L],E/Em[(8.12±1.44)vs.(14.21±4.32)],left ventricular mass index[LVMI,(110.22±42.16)g/m^2 vs.(138.46±34.11)g/m^2]and early-diastolic peak velocity deceleration time(DT)[(192.36±52.68)s vs.(204.11±50.12)s]in HFpEF group(P=0.001 all).Pearson correlation analysis indicated that serum NT-proBNP level was significant positively correlated with E/Em and LVMI(r=0.654,0.755),and significant inversely correlated with LVEF(r=-0.803,P=0.001 all).Conclusion:Serum NT-proBNP level is significantly correlated with E/Em,LVMI and LVEF.So detection of serum NT-proBNP level possess certain auxiliary diagnostic value for HFpEF.
