摘要
目的探讨血清糖类抗原125(CA125)、糖类抗原CA153、肿瘤特异度生长因子(TSGF)及血浆D-二聚体联合检测在乳腺癌诊断中的应用价值。方法选取我院2016年1月~2019年9月经病理证实的乳腺癌患者70例(乳腺癌组)及40例乳腺良性病变患者(良性病变组)作为研究对象,另选取同期40例健康体检者作为对照组,比较三组血清CA125、CA153、TSGF及血浆D-二聚体(D-D)水平及其联合检测诊断乳腺癌的效能。结果乳腺癌组血清CA125、CA153、TSGF及血浆D-D水平高于良性病变组及对照组,差异有统计学意义(P<0.05);良性病变组与对照组血清CA125、CA153、TSGF及血浆D-二聚体水平比较,差异无统计学意义(P>0.05)。晚期乳腺癌患者血清CA125、CA153、TSGF及血浆D-二聚体水平高于早期患者,差异有统计学意义(P<0.05);四项指标联合检测诊断乳腺癌的灵敏度﹑特异度﹑准确度﹑阳性预测值及阴性预测值高于各单项检测,差异有统计学意义(P<0.05)。结论血清CA125、CA153、TSGF及血浆D-D对乳腺癌的诊断具有重要价值,联合检测可以提高乳腺癌诊断的敏感度和特异度。
Objective To explore the value of combined detection of serum carbohydrate antigen 125(CA125),carbohydrate antigen CA153,tumor-specific growth factor(TSGF)and plasma D-dimer in the diagnosis of breast cancer.Methods 70 patients with breast cancer(breast cancer group)and 40 patients with benign breast lesions(benign lesion group)confirmed by pathology from January 2016 to September 2019 in our hospital were selected as the research objects,and 40 healthy medical examiners in the same period were selected as the research objects In the control group,the serum CA125,CA153,TSGF and plasma D-dimer(D-D)levels of the three groups and their combined detection of breast cancer were compared.Results The levels of serum CA125,CA153,TSGF and plasma D-D in breast cancer group were higher than those in benign disease group and control group,the difference was statistically significant(P<0.05);Comparing the levels of serum CA125,CA153,TSGF and plasma D-dimer between the benign lesion group and the control group,the difference was not statistically significant(P>0.05).Serum CA125,CA153,TSGF and plasma D-dimer levels in patients with advanced breast cancer were higher than those in early patients,the difference was statistically significant(P<0.05);the sensitivity,specificity and accuracy of the four indicators combined detection of breast cancer diagnosis the positive predictive value and negative predictive value are higher than those of each single test,the difference was statistically significant(P<0.05).Conclusion Serum CA125,CA153,TSGF and plasma D-D are of great value in the diagnosis of breast cancer.Joint detection can improve the sensitivity and specificity of breast cancer diagnosis.
