老年骨质疏松性股骨粗隆间骨折PFNA与InterTan内固定术后影像学改变的比较 被引量:14

摘要 目的比较股骨近端防旋髓内钉(PFNA)与InterTan髓内钉内固定治疗老年骨质疏松性股骨粗隆间骨折术后影像学改变情况,评价两种内固定物临床应用效果。方法回顾性分析自2016-01—2018-12诊治的195例老年骨质疏松性股骨粗隆间骨折,97例行PFNA内固定(PFNA组),98例行InterTan内固定(InterTan组)。结果PFNA组87例获得随访,InterTan组83例获得随访,随访时间平均13.6(12~18)个月。末次随访时PFNA组11例螺旋刀片切出,4例内固定物松动失效;InterTan组3例螺钉切出,2例内固定物松动失效;InterTan组内固定失败发生率低于PFNA组,差异有统计学意义(P<0.05)。术后3、6、12个月随访时,所有患者均出现了不同程度的髋内翻与股骨颈短缩,PFNA组股骨颈轴角改变程度、股骨颈短缩程度大于InterTan组,差异有统计学意义(P<0.05)。结论InterTan髓内钉具有抗髋内翻、抗股骨颈短缩及术后低内固定失败率的优点,更适用于治疗老年骨质疏松性股骨粗隆间骨折。
