滑膜炎膏治疗膝关节骨性关节炎疗效及对IL-1、IL-6水平影响的临床研究

Clinical Study of Synovitis Ointment in Treatment of Knee Osteoarthritis and Its Effect on IL-1 and IL-6

目的观察滑膜炎膏治疗膝骨关节炎的临床疗效。方法采用随机数字表法将150例膝骨性关节炎患者分为滑膜炎膏组、金黄散组、扶他林组,每组50例。滑膜炎膏组采用滑膜炎膏外用;金黄散组采用金黄散外敷;扶他林组采用扶他林乳膏外用,3组均治疗4周。观察治疗前后3组患者临床症状情况、Lysholms评分、疼痛程度主观评分、生存质量改善情况、关节液中白介素-1(IL-1)、白介素-6(IL-6)水平,评价临床疗效。结果治疗后,滑膜炎膏组的总有效率为92%,高于金黄散组的84%、扶他林组的80%,差异有统计学意义(P<0.05);治疗后,滑膜炎膏组Lysholms评分总分高于金黄散组和扶他林组,差异有统计学意义(P<0.05);滑膜炎膏组疼痛视觉模拟评分法评分均低于金黄散组和扶他林组,差异有统计学意义(P<0.05);滑膜炎膏组WHOQOL-BREF评分高于金黄散组和扶他林组,差异均有统计学意义(P<0.05);滑膜炎膏组关节液中IL-1、IL-6水平均低于金黄散组和扶他林组(P<0.05)。结论滑膜炎膏外敷治疗膝骨性关节炎,能减轻患者临床症状,改善关节功能,提高生存质量,降低炎症因子水平。

Objective To observe the clinical effect of Synovitis ointment in treatment of knee osteoarthritis(KOA).Methods 150 cases of patients with KOA were randomly divided into Synovitis ointment group,Jinhuang powder group and Diclofenac sodium cream group,50 cases each. Synovitis ointment group was treated with Synovitis ointment for external use;Jinhuang powder group was treated with Jinhuang powder for external use. Diclofenac sodium cream group was treated with Diclofenac sodium cream cream for external use,and all 3 groups were treated for 4 weeks. Clinical symptoms,Lysholms scores,subjective scores of pain degree,improvement of quality of life,levels of interleukin-1(IL-1)and IL-6 in articular fluid were observed in the three groups before and after treatment,and the clinical efficacies were evaluated. Results After treatment,the total effective rate of Synovitis ointment group was 92%,which was higher than Jinhuang powder group(84%)and Diclofenac sodium cream group(80%),and the difference was statistically significant(P<0.05);the total score of lysholms in Synovitis ointment group was higher than that in Jinhuang powder group and Diclofenac sodium cream group,and the difference was statistically significant(P<0.05);Visual Analogue Score in Synovitis ointment group was lower than that in Jinhuang powder group and Diclofenac sodium cream group,and the difference was statistically significant(P<0.05);the Whoqol-bref score of Synovitis ointment group was higher than that of Jinhuang powder group and Diclofenac sodium cream group,and the difference was statistically significant(P<0.05);the levels of IL-1,IL-6 in articular fluid in Synovitis ointment group were lower than those in Jinhuang powder group and Diclofenac sodium cream group. Conclusion External application of Synovitis ointment can alleviate the clinical symptoms,improve the joint function,improve the quality of life and reduce the level of inflammatory factors in treatment of KOA.
