摘要
目的:观察姜黄素对兔耳增生性瘢痕的抑制作用。方法:取新西兰大白兔12只,随机分为4组,空白组(A组)、模型组(B组)、姜黄素组(C组)和地塞米松组(D组),除空白组外,其余各组建立兔耳增生性瘢痕模型,瘢痕上皮化后C、D两组分别涂抹姜黄素软膏、地塞米松软膏,B组不予特殊处置。连续用药28 d后对各组瘢痕进行大体观察及组织学观察;考察各组瘢痕的瘢痕增生指数(HI)、成纤维细胞密度(NA);增殖细胞核抗原蛋白(Ki-67)及α-平滑肌肌动蛋白(α-SMA)的表达变化。结果C、D两组与B组比较,瘢痕体积明显缩小,质地变软,颜色变浅,有向A组转化的倾向;HE结果显示,C、D组中胶原纤维排列较B组规则,成纤维细胞(Fb)数目明显减少;HI及NA方面,C、D组明显低于B组,差异具有统计学意义(P<0.05),但C、D两组比较无明显差异(P>0.05);免疫组化结果表明,Ki67及α-SMA在B组中大量表达,而在C、D两组中表达量减少,差异具有统计学意义(P<0.05)。结论:姜黄素能有效改善瘢痕组织内纤维过度沉积的现象,其可通过抑制Fb的增殖及向肌成纤维细胞(MFb)的转化来抑制瘢痕的形成。
Objective:To observe inhibitory effect of curcumin on hypertrophic scar of rabbit ears.Methods:12 New Zealand white rabbits were randomly divided into the blank group(group A),the model group(group B),the curcumin group(group C)and Dexamethasone group(group D).The model of hypertrophic scar was established in the ears of the rabbits.Curcumin ointment was applied in group C and Dexamethasone ointment was used in group D after scar epithelialization.The scar of each group was observed by general observation and histological observation after 28 days of continuous medication.The scar hyperplasia index and fibroblast density,expression changes of proliferating cell nuclear antigen protein(Ki-67)and transmembrane smooth muscle actin(α-SMA)were detected in the groups.Results:Compared to group B,the scar volume decreased significantly,the texture became softer,the color became lighter,and there was a tendency to transform to group A in group C and group D.The HE results showed that in group C and group D,collagen fiber arrangement was more regular than that in group B,and the number of fibroblasts(Fb)was significantly reduced.In terms of hyperplasia index and fibroblast density,group C and group D were significantly lower than Group B(P<0.05);however,there was no statistical difference between group C and group D(P>0.05).Immunohistochemical results showed that Ki67 andα-SMA were highly expressed in group B,but gradually decreased in group C and group D(P<0.05).Conclusion:Curcumin can effectively improve the excessive deposition of fibers in scar tissue and inhibit the formation of scars by inhibiting the proliferation of Fb and the transformation to myofibroblast.
