糖尿病多学科护理团队运作模式研究进展

随着糖尿病患者的逐年增加,其治疗方式及治疗效果受到了越来越多患者的关注。糖尿病患者病情的加重,对患者的生活质量造成了严重的影响。因此,对糖尿病患者进行连续性、系统性的管理已经成了目前社会上所关注的焦点。对慢性病患者进行护理干预时,比较有效的一种方式就是应用多协调的多学科团队管理模式。此种管理模式在经过近些年的应用取得了较好的效果。糖尿病多学科护理团队运作模式不但包括团队的组建、成员职责分工、团队考核、评价机制、质量控制、评价指标的建立等几个方面。该文详细分析多学科护理团队合作模式对糖尿病患者的护理进展。

As the number of diabetic patients increases year by year,its treatment methods and effects have received more and more attention from patients.The aggravation of the condition of diabetic patients has seriously affected the quality of life of patients.Therefore,continuous and systematic management of diabetic patients has become the focus of attention in society.In the late 1990s,when the American Medical Research Center proposed nursing interventions for patients with chronic diseases,a more effective way is to apply a multi-coordinated multi-disciplinary team management model.This type of management model has achieved good results after several years of application.The operation mode of the diabetes multidisciplinary nursing team includes not only the formation of the team,the division of responsibilities of the members,the team assessment,the evaluation mechanism,quality control,and the establishment of evaluation indicators.Since the management of diabetes in China started late,it is extremely important to establish a scientific and standardized management model as soon as possible.
