系统性护理对Miles术后肠造口患者生活质量的影响及护理效果

Effect of Systemic Nursing on Quality of Life of Patients with Ileostomy after Miles and Nursing Effect

目的探讨系统性护理对Miles术后肠造口患者生活质量的影响及护理效果。方法从2018年1月-2019年7月前来该院就诊并接受Miles手术治疗的直肠癌患者中方便选择78例患者作为该次研究的对象,数字法随机分组。对照组患者实施常规护理干预,观察组患者实施系统性护理干预。观察与比较两组患者临床指标、生活质量评分、并发症发生率以及护理满意度。结果观察组患者术后首次肠道排气时间(51.06±4.37)h、术后首次下床活动时间(17.74±3.45)h与住院时间(10.17±2.28)d均明显短于对照组的(88.42±4.61)h、(26.68±3.78)h、(15.62±2.34)d,差异有统计学意义(t=3.851、4.029、3.916,P<0.05)。观察组术后并发症5.13%发生率明显低于对照组28.21%,观察组护理满意度97.44%高于对照组79.49%,差异有统计学意义(χ^2=7.163、7.231,P<0.05)。结论对Miles术后肠造口患者实施系统性护理干预可以提高患者术后生活质量,能够降低术后并发症发生率,缩短首次排气与住院时间,有利于患者恢复,可以取得更为满意的护理效果。

Objective To explore the effect of systemic nursing on quality of life and nursing effect of patients with intestinal stoma after Miles operation.Methods For rectal cancer patients who came to the hospital from January 2018 to July 2019 and underwent Miles surgery,78 patients were conveniently selected as the object of this study,and they were randomly divided into two groups.Patients in the control group performed routine nursing interventions,and patients in the observation group performed systemic nursing interventions.Observe and compare the clinical indicators,quality of life scores,complication rate,and nursing satisfaction between the two groups of patients.Results The first intestinal exhaust time(51.06±4.37)h,the first time to get out of bed(17.74±3.45)h,and the hospital stay(10.17±2.28)d in the observation group were significantly shorter than those in the control group(88.42±4.61)h,(26.68±3.78)h,(15.62±2.34)d,and the difference was statistically significant(t=3.851,4.029,3.916,P<0.05).The incidence of postoperative complications of 5.13%in the observation group was significantly lower than that in the control group,28.21%,and the nursing satisfaction in the observation group was 97.44%higher than that in the control group 79.49%,and the difference was statistically significant(χ^2=7.163,7.231,P<0.05).Conclusion The implementation of systemic nursing intervention for patients with ileostomy after Miles operation can improve the quality of postoperative life of patients,reduce the incidence of postoperative complications,shorten the time of first venting and hospitalization,and help patients recover,achieving satisfied nursing effect.
