TNF-α、IL-6及HE4和CA125在EAOC患者中的诊断价值

Application of TNF-α,IL-6,HE4 and CA125 in diagnosis of EAOC

目的:探讨肿瘤坏死因子(TNF-α)、白细胞介素6(IL-6)、肿瘤标志物人附睾蛋白4(HE4)及糖类抗原125(CA125)在子宫内膜异位症相关性卵巢癌患者中水平变化,并分析其在癌症早期诊断中的应用价值。方法:回顾性分析2014年7月~2018年9月在唐山市妇幼保健院收治的84例卵巢癌患者,其中子宫内膜异位症相关性卵巢癌(EAOC)患者52例,卵巢浆液性腺癌32例;卵巢良性肿瘤患者30例,子宫内膜异位症(EM)患者35例,另外选取同期健康体检妇女50例为对照组。采用微粒子化学发光法检测受检者HE4和CA125含量;酶联吸附实验(ELISA)测定TNF和IL-6含量。结果:HE4、CA125及IL-6和TNF-α在EAOC组的水平低于浆液性卵巢癌组,而高于卵巢良性肿瘤组和对照组(P<0.05);HE4、CA125及IL-6和TNF-α在EAOC组Ⅲ、Ⅳ期的水平明显高于Ⅰ期和Ⅱ期(P<0.05);各指标在低分化组高于高分化组(P<0.05);HE4与CA125和IL-6均呈正相关,相关系数分别为(r=0.415,P<0.05;r=0.362,P<0.05);HE4及IL-6联合诊断EAOC价值最高。结论:炎症因子IL-6和TNF-α和肿瘤标志物HE4及CA125在辅助诊断EAOC方面具有较高价值。

Objective:To investigate the changes of inflammatory factors-tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),human epididymal protein 4(HE4)and carbohydrateantigen 125(CA125)in patients with endometriosis-related ovarian cancer,and to analyze their value in early diagnosis of cancer.Methods:84 cases of ovarian tumor patients were retrospectively analyzed which admitted to Tangshan Women and Children′s Health Hospital from July 2014 to December 2017.Among of them,52 patients were endometriosis-related ovarian cancer(EAOC),32 patients were ovarian adenocarcinoma;30 patients were benign ovarian tumors and 35 patients were endometriosis(EM)and 50 healthy controls were selected.Levels of HE4 and CA125 were detected by chemiluminescence method,and levels of TNF-αand IL-6 were determined by enzyme-linked sorption assay(ELISA).Results:Levels of HE4,CA125,IL-6 and TNF-αin EAOC group were lower than those in serous ovarian cancer group,but higher than those in benign ovarian tumors group and control group(P<0.05);levels of HE4,CA125,IL-6 and TNF-αin stageⅢandⅣof EAOC group were significantly higher than those in stageⅠandⅡof EAOC group(P<0.05);levels in low differentiation group were higher than those in high differentiation group(P<0.05);HE4 was positively correlated with CA125 and IL-6,respectively,which of the correlation coefficients were(r=0.415,P<0.05;r=0.362,P<0.05),ROC curve analysis showed that HE4 and IL-6 combined diagnostic efficiency was higher than that two used alone in the diagnosis of EAOC Conclusion:Detection of IL-6,TNF-α,HE4 and CA125 have high diagnostic value in assistant diagnosis of EAOC.
