COVID-19疫情期间门诊儿童输液流程调整策略及体会

Strategies and Experience of Children Infusion Procedure Adjustment in Outpatient Department during COVID-19 Epidemic

目的探讨COVID-19疫情期间门诊儿童输液流程调整策略的应用效果。方法按照《中华人民共和国传染病防治法》、国家卫生健康委办公厅《关于加强重点地区重点医院发热门诊管理及医疗机构内感染防控工作的通知》、浙江省卫健委《加强医务人员个人防护和实验室检测防护》以及《新型冠状病毒感染的肺炎诊疗方案(试行第五版)》[5]、《新型冠状病毒感染的肺炎诊疗方案(浙江临床经验第一版)》有关感染防控的相关要求,自2020年1月29日起,我院在原有物资、人力的基础上,对门诊重新进行区域规划及输液流程调整,于2020年1月31日正式启动COVID-19疫情期间门诊儿童输液流程调整工作,合理安排人力配置及强化人员培训,树立"以病人为中心"的服务理念,严格以儿童输液室的管理制度及标准操作规程(SOP)为指导,对门诊输液儿童的输液流程进行优化调整。结果科室全体医护人员COVID-19标准防护知识及操作技能知晓率100%;手卫生依从率及正确率,口罩、帽子、隔离衣、防护服的正确穿戴和消毒、隔离及医疗废物处置等医院感染防控措施落实率100%;2020年1月31日至2020年2月7日,门诊儿童输液总量576例次,筛查出发热儿童384例,排查疑似病例7例,确诊病例0例,护理人员感染0例次。结论 COVID-19疫情期间门诊儿童输液流程调整策略有效地控制了新型冠状病毒感染肺炎在医院内交叉感染的发生及传播,为门诊输液儿童COVID-19的临床防控提供了依据。

Objective To investigate the effect of adjusting strategies for children’s transfusion in outpatient department during the outbreak of COVID-19.Methods According to the requirements of the Law of the People’s Republic of China on the Prevention and Treatment of Infectious Diseases,Notice on Strengthening the Management of Fever Clinics in Key Hospitals in Key Areas and Infection Prevention and Control in Medical Organizations issued by the General Office of the National Health and Health Commission of the People’s Republic of China,Strengthening the Individual Protection and Laboratory Testing Protection for Medical Personnel issued by Health Commission of Zhejiang Province,Guideline for Diagnosis and Treatment of Novel Coronavirus Pneumonia(fifth trial version),Guideline for Diagnosis and Treatment of Novel Coronavirus Pneumonia(Zhejiang clinical experience first edition),from January 29,2020,based on the original materials and manpower,our hospital adjusted the function zonings and transfusion process in outpatient department.On January 31,2020,the adjustment work under COVID-19 epidemic was officially launched.We rationally arranged manpower allocation and strengthened staff training,established a"patient-centered"service concept,and strictly followed the management standards and standard operating procedures(SOP)of the children transfusion to optimize transfusion process for outpatients.Results All the medical staff in the department were 100%aware of the COVID-19 standard protection knowledge and operating skills.Rate of hand hygiene compliance,rate of correct wearing mask,isolation gown,protective garment,rate of disinfection,and implementation rate of hospital infection prevention and control measures were 100%.From January 31,2020 to February 7,2020,a total of 576 children outpatient transfusions were performed.384 children with fever were screened,with 7 suspected cases,0 confirmed cases,and 0 nursing staff infections.Conclusion During the COVID-19 epidemic,adjustment of the children outpatient transfusion process can effectively control the cross infection of COVID-19 in hospital.It provides a reference for clinical control of COVID-19 among children getting outpatient transfusion.
