摘要
目的:比较IOLMaster 700、Pentacam HR与OPD-ScanⅢ测量白内障患者术前角膜曲率和角膜散光的差异性和一致性。方法:收集白内障患者40例(60眼)。术前所有患者均接受IOLMaster 700、Pentacam HR及OPD-ScanⅢ检查,测量患眼水平角膜曲率(K1)、垂直角膜曲率(K2)、角膜散光及轴向,计算平均角膜曲率(Km)、散光矢量J0和J45。三种检查结果的测量差异采用配对样本t检验,相关性采用Pearson分析,一致性采用Bland-Altman法分析。结果:三种仪器测量K1、K2、Km、J0和J45,比较差异均无统计学意义(P>0.05),所有参数均具有较高的相关性(P<0.01)。IOLMaster 700与Pentacam HR两者测量K1、K2一致性好;而OPD-ScanⅢ分别与IOLMaster 700和Pentacam HR比较,测量K1、K2的一致性较差;IOLMaster 700、Pentacam HR与OPD-ScanⅢ对J0和J45的测量具有较好的一致性。结论:在测量角膜曲率时,IOLMaster 700与Pentacam HR一致性好,变异范围小,可以考虑替换。而涉及散光测量时,虽然三种设备具有良好的一致性,仍需根据实际情况、权重进行选择。
Objective:To evaluate the differences and consistency in preoperative keratometry and corneal astigmatism in cataract patients measured by IOLMaster 700,Pentacam HR and OPD-Scan Ⅲ.Methods:The clinical data of 40 cataract patients(60 affected eyes)were collected.All patients received IOLMaster 700,Pentacam HR and OPD-Scan Ⅲ examination before surgery.The horizontal keratometry(K1)and vertical keratometry(K2),Corneal astigmatism and axial,the average keratometry(Km),the astigmatism vectors J0 and J45 were calculated.The results were analyzed by the paired samples t test,Pearson coefficient,and Bland-Altman analysis.Results:There were no significant differences in K1,K2,Km,J0 and J45 among IOLMaster 700,Pentacam HR and OPD-Scan Ⅲ(P>0.05).All the biometric parameters showed good correlation(P<0.01).The measurement of K1 and K2 showed good agreement between IOLMaster 700 and Pentacam HR.However,when compared with IOLMaster 700 and Pentacam HR,the agreement was not good with the results of K1 and K2 measured by OPD-Scan Ⅲ.The measurement of J0 and J45 showed good agreement among three devices.Conclusion:The agreement between IOLMaster 700 and Pentacam HR in corneal curvature is high with little variance,which can be considered for mutual replacement.The 3 devices can be used interchangeably to measure corneal astigmatism in cataract patients with good agreement,but clinical practical condition should be considered in the selection of the results.
