摘要
目的:探讨CT引导下臭氧联合蝶腭神经节阻滞治疗持续性特发性面部疼痛的临床疗效及安全性。方法:收集45例持续性特发性面痛患者,所有患者均行CT引导下经翼腭窝臭氧联合蝶腭神经节阻滞治疗。采用视觉模拟评分法(VAS)评价疼痛程度,匹兹堡睡眠质量指数(PSQI)评价睡眠质量,计算总有效率,并记录相关并发症。结果:每个患者至少随访6个月,总有效率为95.6%,术前VAS评分为(8.07±1.37)分,术后1周、2周、1月、3月、6月VAS评分分别(2.57±1.46)分(t=19.234,P<0.05)、(2.29±1.38)分(t=19.175,P<0.05)、(1.78±1.62)分(t=18.168,P<0.05)、(1.58±1.69)分(t=18.363,P<0.05)、(0.91±1.52)分(t=23.479,P<0.05),VAS评分较术前明显降低(P<0.05);术前PSQI评分为(16.36±1.78)分,术后1周、2周、1月、3月、6月PSQI指数分别为(6.59±1.76)分(t=14.640,P<0.05)、(6.36±1.64)分(t=15.676,P<0.05)、(6.16±1.38)分(t=19.906,P<0.05)、(5.87±1.62)分(t=20.061,P<0.05)、(5.67±1.51)分(t=21.608,P<0.05),PSQI评分较术前明显降低(P<0.05);术后主要并发症包括术区短期内面部胀痛不适、面部麻木,无颅内感染、出血等严重并发症。结论:CT引导下臭氧联合蝶腭神经节阻滞治疗持续性特发性面部疼痛安全、有效,具有临床推广价值。
Objective:To investigate the clinical efficacy and safety of CT-guided ozone combined with sphenopalatine ganglion block in the treatment of persistent idiopathic facial pain.Methods:45 patients with persistent idiopathic facial pain were collected.All patients underwent CT-guided ozone through pterygopalatine fossa combined with sphenopalatine ganglion block.Visual analogue scale(VAS)was used to evaluate the degree of pain.Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the quality of sleep.The total effective rate was calculated and the related complications were recorded.Results:All patients were followed up for at least 6 months,with a total effective rate of 95.6%.The preoperative VAS score was(8.07+1.37).The VAS scores of 1 week,2 weeks,1 month,3 months and 6 months after operation were(2.57±1.46,t=19.234,P<0.05),(2.29±1.38,t=19.175,P<0.05),(1.78±1.62,t=18.168,P<0.05),(1.58±1.69,t=18.363,P<0.05),and(0.91±1.52,t=23.479,P<0.05),respectively,VAS evaluation the scores were significantly lower than those before operation(P<0.05).PSQI score was(16.36+1.78)before operation,PSQI index of 1 week,2 weeks,1 month,3 months and 6 months after operation were(6.59±1.76,t=14.640,P<0.05),(6.36±1.64,t=15.676,P<0.05),(6.16±1.38,t=19.906,P<0.05),(5.87±1.62,t=20.061,P<0.05),and(5.67±1.51,t=21.608,P<0.05),preoperative PSQI score was significantly lower than that before operation(P<0.05).Postoperative complications included short-term facial pain and discomfort,facial numbness,no intracranial infection,bleeding and other serious complications.Conclusion:CT-guided ozone combined with sphenopalatine ganglion block is safe and effective in the treatment of persistent idiopathic facial pain,worthy of popularization.
