舒芬太尼复合罗哌卡因硬膜外阻滞阶梯式镇痛在初产妇中的应用效果

Application effect of Sufentanil combined with Ropivacaine for epidural anesthesia with step analgesia in primipara

目的探讨舒芬太尼复合罗哌卡因硬膜外阻滞阶梯式镇痛的临床效果。方法选取2017年1月~2019年1月在我院分娩的120例足月妊娠单胎初产妇,以随机数字表法将产妇分为观察组60例,对照组60例。对照组常规给予舒芬太尼+罗哌卡因,观察组根据宫口扩张度,阶段性给予舒芬太尼+罗哌卡因,对比两组产妇视觉模拟量表(VAS)、产程时间、下肢运动神经抑制情况、新生儿阿氏(Apgar)评分。结果两组VAS评分、Apgar评分比较,差异无统计学意义(P>0.05);观察组Bromage评分低于对照组,差异有统计学意义(P<0.05)。观察组产程潜伏期时间短于对照组,差异有统计学意义(P<0.05)。结论舒芬太尼+罗哌卡因的阶段性分娩镇痛效果良好,相较于常规镇痛方法,该镇痛方法对产妇下肢运动功能抑制程度更低,值得在临床上广泛推广。

Objective To investigate the clinical effect of Sufentanil combined with Ropivacaine for epidural anesthesia with step analgesia.Methods From January 2017 to January 2019,120 full-term singleton primiparas delivered in our hospital were selected and randomly divided into observation group(n=60)and control group(n=60).The control group was routinely given Sufentanil+Ropivacaine,while the observation group was given Sufentanil+Ropivacaine in stages according to the dilatation of uterine orifice.The visual analogue scale(VAS),duration of labor,lower limb motor nerve inhibition and Apgar score of newborn were compared between the two groups.Results There were no significant difference in VAS score and Apgar score between the two groups(P>0.05).The Bromage score in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incubation period of labor in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Sufentanil+Ropivacaine has a good analgesic effect for staged labor,compared with conventional analgesic methods,this analgesic method has a lower degree of inhibition on maternal lower limb motor function and is worthy of wide clinical application.
