检验科医务人员职业倦怠感与心理状态相关性

Correlation between job burnout and psychological state of medical staff in laboratory department

目的:研究检验科医务人员职业倦怠感与心理状态相关性。方法:选取2018年4月至2019年4月我院检验科医务人员45例为研究对象,采用自编一般情况问卷、症状自评量表(SCL-90)和Maslach职业倦怠量表-服务行业版(MBI-HSS)进行调查并分析我院检验科医务人员职业倦怠和心理健康水平,探讨两者相关性。结果:45例检验科医护人员MBI-HSS评分为36~84分,平均(63.76±6.53)分,其中情感衰竭和个人成就感以中度为主,占比分别为57.78%和55.56%,情感淡漠以轻度为主,占比62.22%。我院45例检验科医护人员SCL-90量表躯体化、强迫、人际敏感、抑郁、焦虑、敌意、恐怖、偏执、精神病性评分及总分均明显高于全国常模(t=3.765,2.293,2.431,2.257,5.217,5.332,2.557,2.815,3.385,3.939;P<0.05)。Pearson积差分析显示,MBI-HSS量表情感衰竭评分与SCL-90量表躯体化、强迫、抑郁、焦虑、敌意、恐怖、偏执、精神病性评分及总分呈明显正相关性(r=0.589,0.700,0.561,0.666,0.394,0.446,0.386,0.395,0.762;P<0.05);情感淡漠评分与SCL-90量表躯体化、强迫、抑郁、焦虑、恐怖、偏执、精神病性评分及总分呈明显正相关性(r=0.410,0.730,0.424,0.692,0.813,0.297,0.561,0.645;P<0.05);个人成就感评分与SCL-90量表躯体化、强迫、焦虑、恐怖、偏执、精神病性评分及总分呈明显负相关性(r=-0.384,-0.579,-0.584,-0.856,-0.508,-0.628,-0.675;P<0.05);职业倦怠总分与SCL-90量表躯体化、强迫、抑郁、焦虑、敌意、评分及总分呈明显正相关性(r=0.372,0.367,0.526,0.314,0.386,0.348;P<0.05)。结论:我院检验科医护人员普遍存在职业倦怠感和心理健康问题,且二者之间存在一定相关性,降低职业倦怠感可能对改善医护人员心理健康具有积极作用。

Objective:To study the correlation between job burnout and psychological state of medical staff in laboratory department.Methods:We selected 45 medical staff in laboratory department of our hospital from April 2018 to April 2019 as subjects of the study,used the self-designed general situation questionnaire,symptom checklist scale(SCL-90)and Maslach burnout inventory-human service survey(MBI-HSS)to investigate and analyze the job burnout sense and psychological health level of medical staff in laboratory department of our hospital,and explore the correlation between them.Results:The MBI-HSS scores of 45 medical staff in laboratory department were 36~84 points with an average of(63.76±6.53)points,and the emotional exhaustion and personal achievement were mainly moderate(accounting for 57.78% and 55.56% respectively),and apathy was mainly mild(accounting for 62.22%),and the score of somatization,coercion,interpersonal sensitivity,depression,anxiety,hostility,terror,paranoia,psychosis,the scores of SCL-90 and the total score were significantly higher than the national norm(t=3.765,2.293,2.431,2.257,5.217,5.332,2.557,2.815,3.385,3.939;P<0.05).Pearson product moment analysis showed that the emotional exhaustion score of MBI-HSS scale was significantly positively correlated with the scores of somatization,coercion,depression,anxiety,hostility,terror,paranoia and psychosis and total score of SCL-90 scale(r=0.589,0.700,0.561,0.666,0.394,0.446,0.386,0.395,0.762;P<0.05).The score of apathy was significantly positively correlated with the scores of somatization,coercion,depression,anxiety,terror,paranoia and psychosis and total score of SCL-90 scale(r=0.410,0.730,0.424,0.692,0.813,0.297,0.561,0.645;P<0.05).The score of personal achievement was significantly negatively correlated with the scores of somatization,coercion,anxiety,terror,paranoia,psychosis and total score of SCL-90 scale(r=-0.384,-0.579,-0.584,-0.856,-0.508,-0.628,-0.675;P<0.05).The total score of job burnout was significantly positively correlated with the scores of somatization,coercion,depression,anxiety,hostility and total score of SCL-90 scale(r=0.372,0.367,0.526,0.314,0.386,0.348;P<0.05).Conclusion:There is a general problem of job burnout and psychological health among medical staff in our hospital,and there is a certain correlation between them.Reducing job burnout has a positive effect on improving the psychological health of medical staff in laboratory department.
