新型冠状病毒肺炎患者无创正压通气应用策略及注意事项

摘要 新型冠状病毒肺炎(简称新冠肺炎)疫情发生以来,国家卫生健康委员会诊疗方案中提出重型、危重型病例的呼吸支持治疗原则,应依据患者氧合指数(PaO2/FiO2)、呼吸窘迫程度、胸部影像学情况,选择合适的氧疗、无创正压通气(NIPPV)、有创机械通气(IPPV)的呼吸支持方式.从临床救治工作反馈得知,一线医务人员在管理重症NIPPV时操作经验缺乏,导致临床应用失败率较高.为进一步规范NIPPV的临床应用,我们归纳了新冠肺炎患者NIPPV临床应用策略、操作流程及失败原因等,旨在普及和提高临床一线医务人员NIPPV的操作与应用. Since the outbreak of corona virus disease 2019(COVID-19),the National Health Commission's diagnosis and treatment plan states that the principle of respiratory support for severe and critical cases should be based on the patient's oxygenation index(PaO2/FiO2),respiratory distress degree and chest radiology,and appropriate oxygen therapy,non-invasive positive pressure ventilation(NPPV),invasive positive pressure ventilation(IPPV)respiratorx support could be selected.According to the feedback from clinical treatment,due to the less of experience of management for severe NIPPV,the failure rate of applications was high.In order to standardize the clinical application of NIPPV,we summarized the clinical application strategies,operating procedures,and failure causes of NIPPV,so as Io popularize and improve the operation and application of NIPPV by medical staff.
