国医大师李佃贵治疗功能性消化不良用药规律分析

Analysis of the Medication Rules of Functional Dyspepsia Treated by Chinese Medicine Master Li Diangui

目的:应用中医传承辅助系统软件,分析国医大师李佃贵治疗功能性消化不良用药规律。方法:整理2017年6月至2018年9月李佃贵教授门诊接诊功能性消化不良患者的病案,将患者初诊处方录入中医传承辅助系统(V2.5),采用频数分析、高频药物归类分析、关联规则及基于熵聚类的分析方法,分析李佃贵教授的用药经验及用药规律。结果:符合纳入标准166例患者,共包含166首处方,涉及108味中药,药物主要归经为胃经、脾经及肝经;药物频次≥30次共36味;药物模式支持度个数≥75共23组;规则分析置信度≥0.9共21组;基于熵聚类分析,设置相关度为10,惩罚度为2,得出由柴胡、香附、栀子、乌药、百合、白豆蔻等药物相互组合的11条核心药物组合,5首新方组合。结论:李佃贵教授治疗功能性消化不良以化浊解毒中药为主,佐以化痰散结、理气消积、健脾消食、解郁安神、活血通络之品。

Objective:To analyze the medication rules of functional dyspepsia treated by Chinese medicine master Li Diangui by using the software of Chinese Medicine Inheritance auxiliary system.Methods:The medical records of the patients with functional dyspepsia received by Professor Li Diangui in the outpatient department from june 2017 to september 2018 were sorted out,and the patients'initial diagnosis prescriptions were entered into the Chinese Medicine Inheritance auxiliary system(V2.5).Frequency analysis,high-frequency medicine classification analysis,association rules and entropy clustering analysis methods were used to analyze Professor Li Tiangui's medication experience and medication rules.Results:166 patients met the inclusion criteria,including 166 prescriptions,involving 108 traditional Chinese medicines,mainly including stomach,spleen and liver channel tropism;there were 36 kinds of Chinese medicine with medicine frequency≥30;there were 23 groups with medicine model support≥75;there were 21 groups with rule analysis confidence≥0.9;based on the entropy cluster analysis,set the correlation degree to 10,and the punishment degree to 2,and got 11 core medicine combinations and 5 new combinations which were composed of Chaihu(Chinese thorowax root),Xiangfu(nutgrass galingale rhizome),Zhizi(cape jasmine fruit),Wuyao(combined spicebush root),Baihe(lily bulb),Baidoukou(cardamon fruit).Conclusion:Professor Li Diangui's treatment of functional dyspepsia is mainly based on the Chinese medicine of resolving turbid and detoxifying,with the medicine of resolving phlegm and resolving stasis,regulating qi and eliminating accumulation,strengthening spleen and eliminating food,relieving depression and tranquilizing mind,activating blood and dredging collaterals.
